The educator did not have a child with a life saving medication listed on the required posting.
The assistant did not have documentation of completing evacuation drills on a monthly basis and reported they have not been completed on a monthly basis since last year.

In the bathroom, the following items were accessible to the children in drawers under the sink; ointments, gloves, plastic bags and Febreze spray. In the playroom, the following items were accessible to the children in the cubbies, sunscreen, plastic bags and diaper cream.

In the outdoor space, three trampolines were observed to be accessible. Trampolines by child care children are prohibited.

The progress reports were not available for review during the visit. The Certified Assistant reported the Educator had the progress reports with her.
The Educator did not have proof of current progress reports for the enrolled children.

Progress reports for privately enrolled children were not current. The Educator also stated the progress reports for the child enrolled with the system were given to the home visitor yesterday, so Licensor was unable to view those progress reports.

The Emergency Cards in the First Aid kit did not reflect all of the children enrolled in the program.
The Educator stated that she recently took CPR and First Aid training and had an email, however the date the class was taken was not documented in the email. Please send proof of current CPR and First Aid certification.

Records were reviewed for thirteen children. Four children needed proof of medical documentation. Several children needed Annual Update forms.
The Educator has not been completing the required evacuation drills and logging them. The last records evacuation drills were completed in October 2018.

None of the children had proof of current progress reports. A couple of the children were newly enrolled, so they did not require progress reports yet.

All of the children's records were reviewed. All of the enrolled children required an updated/an or completed progress report.
The assistant staffing the program was unable to find the evacuation log for licensor to review.

Five children did not have annual update forms, two children did not have a date of admission on their enrollment, five children did not have proof/results of lead testing, four children require an updated physical, three children did not have proof of immunization records and one child's enrollment packet was not fully completed.
The parent handbook was missing the emergency plan section.
The first aid kit did not have an approved CPR mouth guard. Several of the children's emergency cards of the children enrolled were not included in the first aid kit.
The staff is not utilizing a second surface during diaper changing. The changing pad being used was ripped and not in good repair.
The mattress being utilized in the crib did not fit the crib appropriately and a cardboard box was being used under the mattress.
The emergency preparedness plan was not in the parent handbook provided to the licensor during the visit.
The refrigerator temperature was 46 degrees. The temperature must measure between 32-40 degrees.
In the lower level kitchen, the trash did not have a lid exposing the garbage. In the bathroom, the baseboard heater was rusted.
There were no carbon monoxide detectors in the licensed space able to be tested.
In the playroom on the half walls there was chipping and peeling paint.
During the visit, the licensor was informed the licensee was no longer residing in the home on a full time basis and was also residing out of state.

The educator is required to complete thirty hours of ongoing professional development. The educator has 18 hours of ongoing professional development completed.

The stairs were accessible to the children . Discussed that when there are children inder the age of 3 years old, the stairs must be inaccessble.
