Five of the children’s records were reviewed. Three children need an updated physical and one child needs a physical. One child’s individual health care plan was expired.
The educator is not alternating exits on a monthly basis when conducting evacuation drills.
The required posting did not include one child with a food allergy.

Records were reviewed for for the 9 children present on the day of the visit. Three children were missing proof of current medical documentation.
There were three children with allergies to medications. These allergies were not posted.
This monitoring outside differential licensing cycle visit inspection recorded no violations or advisories.
Sarah Lucius was caring for a child on the first floor level by herself; she is no longer approved as an EEC assistant. The Educator’s daughter was caring for children on the second floor level of the home; she is also not an EEC approved assistant.

Space - Toxic Substances
The were Lysol wipes and hand sanitizer in the entry way accessible to children as they enter and exit the program. Discussed putting these items on a small shelf out of the children's reach.
Licensed Space
There were 2 persons on the premises who did not have approved background record checks. Sarah Lucius used to be an approved assistant,but her certification expired in September, 2021. Rachel Demers has a LEAD account but never submitted her application.
There were 13 children with the Educator and the certified assistant.
Attendance records did not reflect all of the children on the premises on the day of the visit.
There were children on the second floor level of the home with the Educator’s (REDACTED) daughter. The second floor of the home is unlicensed space.
