The two school aged children were on a separate floor from the educator without the parental authorization form.
The educator did not provide direct supervision for the two school aged children and did not have the parental authorization form completed.

The Educator reported she has not been completing the monthly evacuation drills. Discussed the drills must be completed every month from alternating exits and then documented on the Evacuation/Smoke Detector Log.
There were two plant stands int he basement that were tipping hazards. There were sharp corners along the knee-wall edge.
