All of the children's records were reviewed, one child needs an annual update form, one child needs proof of a physical and one child needs proof of lead testing.
There were several cords longer than 6 inches through out the space which were not secure. A tub of Aquaphor and gloves were accessible to the children under the diaper changing table.
The educator did not have a barricade at the bottom of the stairway.
The educator has the required thirty hours in ongoing professional development but does not have the ten required hours in diverse learners.

Several electrical outlets throughout the licensed space were not covered. The oven knobs in the kitchen were accessible to the children. The staircase leading to the second floor does not have a gate securing the stairway. In the outdoor space, there were several branches with thorns coming into the deck space being used.

The First Aid Kit was missing scissors, a gauze roll, bandaids, and a CPR mouthguard.
There are 2 shelves in the enrichment center that are tipping hazards. There were 2 loose electrical cords in the enrichment center accessible to children.
There are ladders, a lawn mower, and other such equipment on the side yard and a trampoline parts in the backyard accessible to the children. There is a large boulder in the back yard that children are not allowed to climb on.

One of the children does not have a completed progress report. Two of the children do not yet qualify for a progress report based on their enrollment date.
All of the children’s records were reviewed. One child did not have proof of lead testing and one child did not have a date of admission documented.
The first aid kit was missing two of the children's emergency medical cards.
One child was observed to be drinking a bottle without being held.
In the indoor space, the following was observed, the educator did not have a gate barricading the stairs to the upstairs. An air freshener was accessible in the dining room, several ants were observed to be on the floor of the sunroom and in the kitchen the oven knobs were accessible. In the outdoor space, the following was observed a bench press with a bar, an uncovered grill, a ride on lawn mower and several cardboard boxes pilled up.
The educator had a loose fitting sheet on the infant's mattress. The educator did not have a fitted sheet for the child.
