A review of four parental medication consent forms indicated that three were not fully completed to include dates medication is to be given, the possible side effects, and medication storage directions.

It was observed in the GROSS MOTOR room that there was a small holed in a window screen and there was chipped and peeling paint around the room. The program is in lead paint compliance. The Program Director stated that it was due to toys hitting the walls during play.
It was observed in the PRESCHOOL classroom that at least four children's water bottles were not labeled with the child's name for whom they are intended.

It was obesrved that in the EXPLORERS classroom, there was an electrical cord to a speaker not secured and accessible to children.
There was no evidence of a current fire inspection. An inspection was completed but corrections needed to be made prior to issuance.
It was observed that in the EXPLORERS classroom, there was chipped paint in the Dramatic Play area.

INFANT A and INFANT B - It was observed that the refrigerators did not have a thermometer verifying temperatures between 32-40 degrees.
The following was observed and accessible to children: ALL CLASSROOMS - The windows did not have screens in good repair. The Licensee is aware and in the process of having them fixed. GROSS MOTOR - The vinyl rainbow mat had a large tear across it.

One Educator was overheard speaking to preschool-aged children in a negative tone stating "you're not listening, get in line" and "you don't get go to what ever you want" while the group was transitioning from the classroom to the playground.
In the front Preschool classroom, it was observed that multiple electrical cords were unsecured and accessible to children next to the fish tank.

In the Toddler/Preschool classroom (Explorers), it was observed that four children were signed in on the attendance sheet and there were five children present. This was immediately corrected by staff and the number of children present was verified.

Staff and children wash hands: before eating or handling food
A review of the staff meeting log indicated that the meeting times was not documented for meetings 2023-2024.
The following was observed and accessible to children: Outside the building's exit, there was a snow shovel, push broom, rake, and vegetation growing through the fence. The cleaning tools were removed and stored away during the visit. The fence separating the Toddler and Preschool outdoor play areas had rust along its top. On the Preschool climber stairs, there was exposed rust on the top step.
It was observed that evacuation procedures were not posted at the secondary exits of the WOBBLERS and KINDERGARTEN PREP classrooms. The Licensor was informed that the program had recently had its walls repainted and was instructed not to post items until a week after completed.
A review of classroom first aid kits indicated that three did not contain all required items such as CPR mouthguards, scissors, or tweezers.
The Building Inspection expired on June 30, 2021. The Licensee stated that they have been in communication with the Lakeville Building Department and they are behind in inspections.
