It was observed that a current staff schedule was not posted.
A review of six children's records indicated the following: 1) Two did not have current enrollment records. 2) Two did not have parent signatures on enrollment records. 3) Three did not have evidence of a current physical examination.

A review of three staff records indicated that one did not have evidence of a current PQR.
It was observed in TODDLER and JR PRESCHOOL that morning snack was set out on the table and uncovered. One child was observed touching and eating another child's food.
It was observed in INFANTS that a thermometer verifying temperature was not located in the refrigerator.
It was observed in PRESCHOOL that there was chipped paint.
A review of the evacuation drill log indicated that one was not conducted in September.
A review of six medications indicated that two were expired.
A review of five IHCPs indicated the following: 1) All did not identify which staff were designated and trained to administer the medication. 2) One was expired and 3) one did not have a parent's signature.
A review of four medication consents indicated that two did not have a parent's signature.

a review of the staff meeting log indicated that a meeting was not consistently held on a monthly basis.
The following was observed and accessible to children: Junior Kindergarten - There was a stapler located in an unlocked drawer, the play kitchen was not secured and tippable, the art shelf was not secured and tippable, and the broom and dustpan were located next to the counter. Transitional Kindergarten - There was a broom and dustpan located next to the counter.
A review of three individual health care plans indicated that all three did not include all the required information needed.

The following were observed and accessible to children: BOYS BATHROOM - In the first stall along the bottom edge of the right wall, there was rust and peeling paint. PRESCHOOL - In the Dramatic Play area, the pink plastic baby set was not secure.
The following were observed and accessible to children: HALF-DAY PRESCHOOL/SCHOOL AGE CLASSROOM: In the unlocked cabinet underneath the hand washing sink, there were bleach wipes, plastic trash bags and a box of gloves. THROUGHOUT THE PROGRAM: There were purses located in unlocked cabinets.

A review of five staff records indicated that one did not have evidence of a current physical examination, one did not complete the EEC Essentials 2.0 training prior to working directly with children, and one did not have evidence of bi-monthly observations since July 2023.
It was observed that there was rust on the side of one stall in the boys bathroom.
