Food stored in clean/covered containers
During the visit, a preschool educator was observed displaying harsh, hostile behavior toward a group of preschool children. The educator was seen abruptly approaching the children, roughly grabbing a stack of books from the children seated on the rug, and forcefully placing the books onto a chair. The behavior was not caring or nurturing and was inconsistent with expectations for appropriate educator-child interactions. The Director immediately responded by entering the classroom, requesting support, and calmly and respectfully removing the educator.
Not all snacks were stored in covered containers.

During the visit, one staff record was missing evidence of[REDACTED]and physical exams, while another did not have references. Additionally, not all staff records were available for review.
During the visit, toddler groups were outside on the playground. Three educators were standing together and talking. The educators were not positioned to maximize their ability to see or hear the children in their care.

In a toddler classroom, an educator was not positioned to maximize her ability to see and hear children in their care. The Educator was sitting at the table with her back to the children. On the playground, an educator sat on a toybox disengaged from the children.

During the visit the following was noted: 1. The refrigerator in the infant/toddler classrooms needed cleaning. 2. Not all classroom equipment was safely secured. 3. Several children's sofas in the classrooms were ripped and needed to be replaced.

During a visit to the infant classroom, the following was noted: 1. A half wall divides the room. Educators weren't positioned to maximize their ability to see an infant sleeping on the other side of the half wall. 2. One Educator was in the kitchenette Area with was closed off by a door and curtain, leaving another Eductor alone in the classroom with six infants.

The following was noted: 1. Not all classroom equipment was safely secured. 2. The PreK classroom was cluttered. The top of the shelves were diry and sticky. 3. The walls had chipping paint
The following was noted: 1. The area between the poured rubber surfacing and the turf/grass is separating, posing a potential hazard. 2. The black rubber border around the ground surfacing was exposed, causing a potential tripping hazard. 3. Sandbox needed to be repaired 4. There was a broke drying rack on the playgroud.
This investigation inspection recorded no violations or advisories.
Not all children were observed washing thier hands after using the bathroom.

The following was observed: 1. Not all electric wires were out of children’s reach. 2. There was chipping paint in several classrooms throughout the program. 3. There was poor ventilation in the Preschool II classroom.

Outdoor Space - R Apr 2025
During the visit, an educator left the toddler classroom with one child, leaving another educator out of ratio caring for eight children.
During the visit, an educator guided a child out of the classroom and to the Director's office due to challenging behaviors. The educator did not use positive guidance techniques such as setting clear and consistent limits and redirecting. Immediately after the educator arrived with the child at the Director's office, the Assistant Director gently directed the educator to return to the classroom with the child.
During the visit, the Toddler Four Classroom did not have a sufficient quantity and variety of materials and equipment to engage all children present.
The area between the poured rubber surfacing and the turf/grass is separating, posing a potential hazard. The black rubber border around the ground surfacing was exposed, causing a potential tripping hazard. A hose was left out on the playground, causing a potential tripping hazard for children.
Equipment is clean, safe, secured, and poses no hazard.

The following was noted: 1. Not all classroom equipment was safely secured. 2. One of the preschool classrooms was cluttered. 3. The fabric of a child-size couch in a classroom was torn.
Water bottles intended for individual use by children were not all labeled with the name of the child for whom they are intended.
