There wasn't evidence of staff observations.
At the time of the visit, the following was observed: 1. Ceiling tiles were in disrepair. 2. Exposed installation was found in toddler II’s closet, where sleep mats are stored. 3. Multiple classroom fan cords were found dangling and not secure to the walls.
In the preschool room, thumbtacks were stored in an unsecured drawer accessible to children.
