It was observed that there was an(REDACTED) sitting up and playing in a pack and play. It was observed that there was a blanket on the mattress and not a sheet. The blanket was not observed to be loose fitting; however, it was a blanket. The educator indicated that she does have a fitted sheet for that pack and play, however the infant has sensitive skin and has been having a reaction; of which appears to be from the fitted sheet. Parent was concerned that infant's face was always red and requested to have the educator not place infant on the fitted sheet. The fitted blanket observed, according to the educator has worked and is not giving the child a reaction. Educator did remove blanket and placed fitted sheet, which licensor observed that it was readily available and appropriate fit. It was discussed with the educator that communication between parents and the educator should be documented regarding plan for child's sensitive skin. If there is documentation from doctor indicating specific plan for materials used for this infant during naptime, this should be in the child's file.
