At the time the licensor visited the preschool classroom one teacher was alone with 13 children. Approximately 2-3 minutes later the second teacher returned to the classroom. The ratio for preschool/school-age groupings is 1 teacher to every 10 children.

At the time of the visit five child files were reviewed and the following was found: - two files contained expired physical - one child's file did not contain their IHCP form, but was instead being stored in the classroom with their emergency medication
At the time of the visit four staff files were reviewed and the following was found: - all four files needed current PQ registries - two files were missing current physicals within the last two years - one file was missing the educator's BRC suitability letter - one file did not include the educator's teacher certification
The second-floor preschool classroom had furnishings in the dramatic play area that were not properly secured to the floor or wall in order to prevent possible tipping.
Staff personal belongings were on the floor, accessible to children, in the first-floor kindergarten classroom.
This monitoring visit inspection recorded no violations or advisories.
At the time of the visit two children with emergency medical conditions were reviewed and the following was found: both IHCP forms were missing some of the EEC required information.
At the time of the visit there was a toilet brush and plunger in the Pre-K/K bathroom, accessible to the children.

Attendance in the Pre-K classroom was not accurate on 5/31/22. One child who had arrived late to the program was not market as present on the attendance list.

A review of one staff file indicated that the record did not include; evidence of immunity to MMR, documentation of completed required reference checks, and documentation of a staff orientation.
The following was observed and accessible to children: Bathroom (1st Floor) -A plunger and toilet bowl cleaning wand were stored next to the toilet. The janitor's closet in the bathroom was unlocked and there were six cleaning agents, mop, and mop bucket accessible. Pre-K Classroom -Two play kitchens and one bookshelf were not secure. -A laptop charging cord was plugged into a desk within reach of children. Kindergarten Classroom: -One storage unit was not secure. Bathroom (2nd Floor) -Two toilet bowl cleaning wands were being stored next to the toilet.

At the time of the visit two children with emergency medications were reviewed and the following was found: one did not have all their prescribed medication onsite and the other had an expired medication.
A review of 3 staff records indicated: -Two staff did not have evidence of bi monthly observations every other month as required. The two observations that were available included all 3 staff observed in the classroom. They were not individualized for each staff person. In addition, the staff did not sign the observations that were recorded to indicate that they had been consulted with.
The program did not have documentation for staff meetings for December 2019.
The BRC consent for one staff was not complete and did not indicate when the consent was completed.
The Kindergarten classroom attendance did not accurately reflect the number of students in attendance. In addition, in the Preschool room, the teacher did not have a method to show who she was assigned to supervise at any given time. When asked, the group had split. Part of the group was in the classroom and the other part of the group was in a specialty class upstairs. The list that the teacher in the preschool room had showed all 18 children assigned to the classroom for the day instead of the 9 she was assigned to supervise at the time.
The following were observed: -Throughout the space, shelves were tippable and Clorox wipes and hand sanitizer were accessible to children. -Blind cords were accessible in one room. -A Swiffer wet jet was accessible in the preschool room. A cord and string of lights over six inches was accessible to children. There was chipped paint and chipped plaster in the preschool room.
There were 5 electical panels, marked “caution- danger of electrical injury” accessible to children on the playground. In addition the down spout was disconnected.
