At the time of the visit the building and fire inspection were expired. The licensee went to the office to pay fee and has a visit scheduled for 1/9/26.
At the time of the visit three children with emergency medical conditions were reviewed and the following was found: one of a child's medications was expired.

At the time of the visit there were three classrooms (rooms 1, 2 and 6) that had diaper changing stations that do not have direct access to running water for hand washing purposes. Two of these three rooms (rooms 1 and 6) had portable sinks in them that had been purchased due to a citing of this same nature made back in February 2025. The sinks had been purchased as noted in a non-compliance response, however never actually installed and used. The third room (room 2) was reported to have had a portable sink ordered for it however there was not one in the classroom.

At the time of the visit three children requiring emergency medications were reviewed and the following was found: one child had an expired medication.
At the time of the visit the toddler room and the first steps 1 classroom did not have any method of handwashing after diaper changing. Handwashing stations should always be in close proximity to the diaper changing area for immediate use.
At the time of the visit adult and child hands were not being immediately washed after diaper changes in the toddler and first steps 1 classroom.
At the time of the visit four child files were reviewed and the following was found: one child's transportation form was missing a parent signature.

Enforcement and Compliance with Regulation
On 4/14/2022, the licensor arrived to the program to conduct an unannounced monitoring visit. Upon entering the program, the licensor observed Educator A and the Program Director in the program's office. When the licensor entered the office, Educator A left and returned to a classroom. The Program Director met with the licensor for approximately ten minutes. The Program Director requested the licensor meet in the infant room at the end of the visit as they would be covering in the infant classroom. Upon entering the infant classroom, the licensor observed Educator A caring for five infant children. The licensor asked the Program Director and Educator A if there was an educator in the infant classroom while they were meeting in the office. They stated that Educator B had been covering in the infant classroom. The licensor stated that the classroom was out of ratio as the educator to child ratio for infant aged children is 1:3 and there were five infants present. As the visit proceeded, the licensor met with Educator B who stated that they arrived to the Preschool classroom around 7:45am on this date. Educator B stated that they had been in the Preschool classroom all morning and was not covering in any other classroom. The licensor returned to the infant room and asked the Program Director if any educator was present in the infant classroom while they were meeting in the office. Educator A stated "I asked Educator B to cover in the classroom but they forget things easily". Educator A then stated that they were "unsure" if anyone came to cover the classroom as they had only left the classroom with the infants for "two minutes". There is sufficient evidence to conclude that five infant children were left alone in a classroom for an unknown amount of time while the Program Director and Educator A met in the office. The office is approximately twenty-five paces away from the classroom. The infant classroom door was closed when the licensor arrived to the program. During the visit, the program's attendance records in the infant classroom did not indicate when children left the program on 4/13/2022. The program's attendance records in the infant classroom did not indicate when children arrived to the program on 4/14/2022.
The provider presented misleading information to the Department of Early Education and Care. On 4/14/2022, the licensor requested to review the infant classroom attendance during a monitoring visit. The staff stated, "I am going to have to take the hit for this." and handed the licensor an attendance binder with a paper copy of the attendance for review. The attendance that was presented to the licensor for review did not include dismissal times for five infant children on 4/13/2022. The attendance that was presented to the licensor for review did not include arrival times for five infant children who were present during the visit 4/14/2022. The Program Director called the licensor on 4/19/2022 and requested to submit the electronic attendance from the Brightwheel Application for review. The Program Director stated that this attendance was maintained on the day of the visit. A review of the attendance submitted on the day of the monitoring visit on paper and a review of the attendance recorded in the Brightwheel Application indicated different recorded attendance times.
A review of the emergency evacuation drill log book indicated that the last emergency evacuation drill occurred on 11/18/2021. The provider must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly.
