At the time of the visit four staff files were reviewed and the following was found: 1. Staff A was in need of a second reference. 2. Staff B needed a copy of their BRC suitability letter and a current physical. 3. Staff C needed their BRC suitability letter. 4. Staff D needed staff information form, resume/application, interview details, PQ registry, orientation form, current physical, and evidence of MMR.

At the time of the visit there were no staff meetings logged for February, March, May and July 2025. Staff meetings should be conducted and logged on a monthly basis.
At the time of the visit four children with emergency medical conditions were reviewed and the following was found: one child did not have a consent form for their prescribed medication.

A review of the evacuation drill log indicated that a drill was not consistently conducted on a monthly basis.
A review of four individual health care plans indicated that all were not fully completed to include information regarding which staff were appointed to administer the medication in case of an emergency and who they were trained by.

Indoor Space
It was observed that in three of the classrooms, there were brooms and dust pans accessible to children.
The following were observed and accessible to children: TODDLER 3 - Next to the counter, there was a broom and dustpan. Was corrected during visit. TRANSITION - Between the children's cubbies, there was a broom and dustpan. Was corrected during visit. -INFANT/TODDLERS - In the Bathroom area, there were children's chairs stacked up; potential cause for a fall hazard. Was corrected during visit.
Equipment is clean, safe, secured, and poses no hazard.

At the time of the visit 5 staff files were reviewed and the following was found: one file was missing a current physical and one file was missing a staff information form.
It was observed that in the bathroom between the Toddler 3 and Transition rooms was not covered. This was corrected during the visit.
A review of one Individual Health Care Plan indicated that information was missing regarding which staff were trained to administer and who were they trained by.
It was observed that in the Toddler 1 and Toddler 2 classrooms, there was chipped paint. The Lead Inspection indicated that the program is in compliance.
