During the visit, a review of six children's records indicated the following: -Two individual children, each diagnosed with (REDACTED) did not have evidence of an Individual Health Care Plan (IHCP). -Two individual children's records did not have complete IHCP's for two children diagnosed with an allergy requiring an(REDACTED) -One child's record did not have a complete IHCP for a medical diagnosis of (REDACTED).
Upon a review of 11 medications stored at the program for six individual children, the following was identified: -One child's prescribed (REDACTED) -There was no (REDACTED) onsite for a child diagnosed with (REDACTED)

The Program administrator is Director I certified. The Program is licensed for 120 Preschool-School Age children, requiring a administrator to be Director II qualified.

Toxic Substances
During the visit, three children's files were files were reviewed. Two files did not contain completed and current Medication Consent and Individual Health Care Plan forms.
The program did not have prescribed (REDACTED)onsite for a child. All other medications onsite were readily available.
Upon a review of 11 Medication Consent Forms, the following was identified: -One child's record did not have evidence of a current Medication Consent Form for prescribed (REDACTED) -One child's record did not have a Medication Consent Form for a prescribed (REDACTED).
-There was evidence of peeling laminate trim on the sink and cabinet area in the Children's House B classroom that is in need of repair. -There were uncovered trash containers in the girls bathroom, boys bathroom, and in the hallway. -In the Media room, a table had evidence of torn and peeling laminate and is in need of repair.
The Building inspection certificate and the Fire Inspection certificate were not current at the time of the visit.
Emergency medications were stored in individual classrooms at the time of the visit instead of with the educators, who were with the children in the media room.
The following hazards were observed during the visit: -Clorox wipes were accessible to children under the sink cabinet in Children's House C classroom. -The girls and boys restrooms had cleaning mops accessible to children. -Children's House A classroom had mops and brooms accessible to the children. -A radio with a wire longer than six inches was accessible to children in the Children's House B classroom. -In the hallway, a large sharp paper cutter was accessible to children.
