The licensor reviewed 3 of the children records. The following were observed: 1.One child record is not up to date. The annual update was due 7/18/25. 2.One child record was not accurate when the child did not have the individual health care plan since the child has Asthma. The same child record did not list all the medication that the child takes. 3.One child record did not specify where the child can go when off the premises (page 5 of the enrollment). 4.One child record was not complete when the following pages were not filled out or complete 2, 4, 6,7, and 8 on the enrollment packet).

The licensor reviewed 3 of the children progress. The 3 progress reports were not available.
The educator did not have the required posting for a child who has asthma.
The licensor observed the following: 1.The educator did not have an individual health care plan for a child who has Asthma. 2.The educator did not have the medication for a child who has Asthma.
The educator did not have the consent to administer medication for a child who has Ashtma.

Exit Safety
The licensor was not able to see the progress reports for the four children that are enrolled. The educator was not able to locate the progress reports.
The licensor observed that the second floor back exit that is part of the licensed space was not free from clutter (boxes, electrical cords, bucket of plaster, and trash) accessible to the children.
Eventhough, the educator stated she is not using the first floor space at this time, the educator did not have the exit on the first floor safe and accessible to the children. At the present time she does not have two separate exits to the outside.
This monitoring visit inspection recorded no violations or advisories.
The water temperature was 125 F degrees exceeding the 120 F degrees required.
Licensor reviewed the Children's Records Checklist completed and submitted by the educator with the renewal application. Additionally, 3 random records of the children currently enrolled were reviewed during the visit, the following was observed: 1) a child enrollment packet was not up to date. Additionally, the developmental history was incomplete in the enrollment packet. 2) two children enrollment packets were not up to date. 3) a child physical examination was not up to date.

This monitoring visit inspection recorded no violations or advisories.
During the visit, Licensor reviewed the educator and Regular Assistant [REDACTED] records. The following was observed: 1) [REDACTED] physical examination was not up to date. Additionally, the PQR information was not up to date, and there was no evidence of completion of the 12-hours of EEC Essentials in the record.
Licensor reviewed the Children's Records Checklist completed and submitted by the educator with the renewal application. Additionally, 3 random progress reports of the children currently enrolled were reviewed during the visit, the following was observed: 1) two children’s progress reports were not up to date.
