During the visit, Licensor observed an equipment with a green slide attached (green slide=21 inches) in the outdoor space, which measured above 18 inches, and the educator did not have a Fallzone as required.
Fallzone
Licensor observed the following indoor safety hazards accessible to the children: 1. Dog’s water plate in the kitchen 2. The trash container in the bathroom is not lined. Licensor observed the following outdoor safety hazards accessible to the children: 1. Uncovered grill and propane gas tank 2. Lawn mower 3. Open shed used for the storage of tools 4. Front gate is missing bars and the open space represents an entrapment hazard

Upon this Licensor’s arrival around 9:30am, [redacted] was in a bouncing seat. The educator kept[redacted] close to her and the other children and offered [redacted] a soft block to play with. 1. The educator removed the child from the bouncing seat at 10:15am to change [redacted] diaper and placed [redacted] back in the bouncing seat.
