The licensor observed the front exit and back exit were not clear of obstructions with the following (front): piles of clothes, luggage, books, paper bags with clothes inside. The back exit is not clear with the following: a pack of water bottles and an empty box.

During the visit, Licensor observed the educator did not have a thermometer inside the refrigerator to verify the refrigeration and storage for food at not less than 32 F, nor more than 40 F degrees.
During the visit, Licensor observed {REDACTED} was present, and the educator stated the assistant works in the program on Thursday's every week. However, the educator did not have evidence of {REDACTED} attendance.
During the visit, Licensor observed the educator did not have Regular assistant {REDACTED} EEC license posted as required.
During the visit, Licensor reviewed the children records checklist completed by the educator. Additionally, 4 random records of the children currently enrolled , the following was observed: 1) a child enrollment packet was incomplete. The packet was missing: acknowledgment of parent handbook, physician healthcare information, developmental history, general permission. Additionally, the child was {REDACTED} results on file. 2) a child physical examination was not up to date. Additionally, the child was missing {REDACTED} results on file. 3) a child was missing the general permission in the enrollment packet. 4) a child enrollment packet was not up to date. Additionally, the child was missing updated {REDACTED} results on file.

During the visit, the educator stated {REDACTED} works in the program once a week (Thursdays). The educator did not have evidence of assistant record/file available.
During the visit, Licensor reviewed the children records checklist completed by the educator. Additionally, 4 random progress reports of the children currently enrolled were reviewed, the following was observed: 1) four children were missing progress reports on file.
During the visit, Licensor observed the educator did not have evidence of an Emergency Preparedness Plan available.
Despite educator having individual labelled toothbrushes, Licensor observed the toothbrushes were stored inside individual plastic containers with a lid. The toothbrushes were no stored in a safe and sanitary manner open to the air.
