Cited finding

The licensor reviewed 4 of the children records. The following were observed: 1.One of the children records was not accurate when the child's medication was not listed. 2.Three of the children records were not accurate when the assistants names were not listed on the general permission page to take the children off the premises ,to transport to emergency facility, and to receive emergency medical treatment. 3.Three of the children records did not specify where the children can go when off the premises.
