The cots in the [redacted] had the fitted sheets on the rest mats while being stacked on top of each other not being stored in a safe and sanitary manner.

This was noted as a non-compliance on the previous visit 10/23/2019 as the program stated the walls with scribbles and marks were wiped down with Clorox wipes. The program is still in the process of scheduling a time for the program to have the walls repainted and cleaned with the scribbles and marks on the walls.

At the time of the visit (2) of the classrooms had the children's food in a cubby not in the refrigerator as stated on the package and (2) refrigerators were not registering between 32-40 degrees.
At the time of the visit the program had had (2) pieces of furniture that needs to be secured to wall, there was clutter on top of the cubbies in the hallway and (1) classroom.
The program is missing March and April for practice fire drill.
The following hazards were accessible and within reach of the children: (2) educator purses, a can of air freshener on the shelf in the classroom, the children's bathroom had soap on the floor making it very slippery, and plastic bags.
At the time of the visit the program had (2) {REDACTED} bottles that were expired.

The following documentation was missing and/or expired from the children's files: (2) Physicals expired, (1) First Aid Emergency Medical Consent form expired, (1) transportation plan expired, and (1) Developmental History expired.
A child was having a difficult time on the playground with their peers and continued to hit the children. The educator did not provide guidance to the child in a consistent way based on the understanding of the individual needs and development of this child.
A closet in the {REDACTED} classroom needs to be fixed and/or replaced as the door hinge has a potential to pinch the children’s fingers.
The educators did not wash their hands after emptying the trash can and the children were not instructed to wash their hands after wiping their nose.
The following hazards were accessible to and within reach of the children: (2) containers of Clorox wipes and plastic trash bags.

The extra snacks in the ziploc bag need to be labeled with the date the bag is opened and when it expires.

On the preschool side of the program there are several areas where the walls are not clean and have marker and scribble marks.
In the program there were (2) medications that needed to be in the original manufacturing packaging.
The educator did not wash their hands in between assisting a child with eating finger food then picking another child up and moving them then going back to assist the child with finger foods wearing the same glove.
The Individual Health Care Plan was missing the following documentation: parent signature with the date, side effects, consequences, who trained the individual to administer the life-saving medication and who is trained to administer the life-saving medication.
The educator did not wash their hands after changing a child's diaper.
Two [MEDICINE] were being stored in a locked cabinet and not easily accessible in case of an emergency.
The following hazards were accessible and within reach of the children: Free breeze bathroom spray and Aqua Phor was in the children's bathroom, trash bags in an unlocked cabinet, several containers of sunblock and bug spray located in the children's cubby and under the changing table in an unlocked cabinet, and the vacuum/mops being stored in the children's bathroom must be removed.
