The licensor reviewed 4 of the children records. The following were observed: 1.One of the children records did not have the new annual update. The last signature from parent was 5/15/23. 2.Two of the children records did not have the general permission to basic transportation filled out (see page 6 of enrollment). 3.One of the children records did not list assistants names as part of the permission to medical facility… (see page 6 of enrollment). 4.One of the children records did not list topical medication/ointments(see page 6 of enrollment). 5.Four of the children records did not include the general permission when off the premises (please see EEC enrollment packet to use as a guide for your enrollment). 6.Four of the children records did not list names of educators as part of the permission to a medical facility.(educator wrote “staff”)
