During the visit, educators did not ensure children washed their hands after eating.
During a review of children's files, a child was missing a written parental consent to administer medication. A child was missing evidence of a current physical.
At the time of the visit, there wasn't evidence of practice evacuation drills occurring monthly.
At the visit, a child's cot was placed in front of the door in the Toddler/Preschool room. The cot obstructed an exit and evacuation routes. A hole was in the door to one of the classrooms on the bottom floor.

A preschool educator was not appropriately supervising children on the playground. The educator was asked twice by the Licensor to ensure she was in close proximity to children out on the playground to be able to intervene quickly when necessary.
The children's bathroom had air freshener spray on the counter, accessible to children. Eductors' coffee cups were within the children's reach in the classroom.
Children were climbing on playground equipment where the protective surfacing was frozen. Frozen surfaces are not considered acceptable impact-absorbing surfacing.

Diapering and Toileting for children under 2.9
Three children's records were randomly reviewed and the following was learned: First aid/emergency medical consent, off site consents, transportation plans, developmental histories were not update yearly as required.
During the visit, children had access to a dehumidifier observed in the center of the classroom. Educators reported that the dehumidifier was needed due to rain water coming in and the carpet becoming damp. Not all classroom equipment was secured as required. In one of the children's bathrooms the following was observed: The children had access to a toilet brush. The bathroom ceiling was stained. The floor tile was chipped causing a potential tripping hazard. The bathroom did not have adequate lighting. The janitor's closet wasn't locked allowing children to have access. An electrical outlet was not covered.
During the visit, an educator was observed not wash her hands between assisting several different children with wiping their noses.
