Children’s files were reviewed. Files did not specify where children are taken when off premises. One file did not have lead screening and was missing signature dates on page 3 of enrollment packet. One file needs the annual update for the enrollment packet and needs physical description on first page of enrollment packet to be completed.

There was a tall lamp in the nap room that was easily tippable. There was access to plastic bags from opened diaper packages and baby wipes were accessible on diapering table.
Educator had a cloth covering over the diapering mat which is not approved, it must be a material that can be cleaned and disinfected after each use.

Supervision
Reviewed children’s files. The general permission for two files did not indicate where children are taken when off premises and two other files had parent signature not giving permission to take children off premises. Educator must take children off premises because her yard is not yet approved for use. One file did not have lead screening. All files did not include physical description of the child and one file had incomplete sections.
It was observed that there was (REDACTED) napping in a separate room behind a closed door.
Ratios
Educator was again out of her ratios during this follow up visit. Educator was present with three infants all under 15 months. Ratios were again reviewed with the educator. Two parents picked up their infants while licensor was still at the visit. Discussed with educator that based on her enrollment she can only have two infants present and her own child who is over 15 months. Educator will provide a schedule.
