It was observed there was minimal construction activity in the front of the home for a new walkway. The temporary fencing to prevent access to hazardous materials was observed to not be completely closed off and was not sturdy in sections.

Children’s records were reviewed. Three files had missing information on first page of enrollment packet; physical description (3), date of admission (1), and age of admission (1). One file did not have lead screening. One file had missing signature date for acknowledgment of parent handbook section.

One child did not have an updated progress report on file.
It was observed that the room in which toddlers were napping did not have adequate lighting for proper supervision. It was discussed with educators that room is too dark.

Assistants were asked for updated progress reports, assistants indicated they did not have access to the reports at that time. Attendance records for children were reviewed. It was observed that there are sporadic days in which children were not signed out, typically on the last day of the week.
There was a cloth covering on the diapering mat. Assistant was observed checking diapers downstairs and there were no individual diapering materials observed.
It was observed that in one of the basement playrooms, there was a portable fan connected and accessible to children. It was observed that the door to an unlicensed room was left open, giving access to that room. There is access to a portable heater and an outdoor structure that is not approved for use.
There are two climbing structures measuring above 18 inches. Swingset does not have 6 foot fall zones from right side and backside, second structure does not have 6 foot clearance from both sides and back side from end of orange colored slide.
There was not posting for child with allergy.
