Educator was out of her ratios during today’s visit. There were five children under the age of 2 years old, which includes one infant present with just one educator. The assistant was not present. Based on attendance and enrollment it was also reviewed that the assistant must be present at the program before the 4th child arrives to the program. Two children were picked up during the visit; one was picked up within 25-30 minutes and the second was picked up within the hour. This brought the educator into ratios at that time.
Educator did not wash children’s hands and her hands after each diaper change.
Educator did not have first aid kit outside during outdoor play.

Educator did not have required posting for child with life saving medication.
A random review of children’s files was completed. It was observed at least three files had incomplete sections throughout the enrollment packet. It was reviewed with the educator that she must review all enrollment packets to ensure there are no sections that are left blank. Two files did not have lead screening. One file indicated there was a life saving medication prescribed to the child and there was no individualized healthcare plan on file.
Educator does not have consent signed by parent to administer medication and the life saving medication was not on the premises.
It was observed that the educator had an outdoor climbing structure inside. This structure is not approved for indoor use. There was a portable fan in the space where children nap. It was observed that there was access to hazards within the outdoor space. Hazards observed were multiple gardening tools and products, including a shovel. There were storage shelves outside with multiple items that included gardening hazards, these shelves must be better secured to ensure there is no tipping.
A barrier needs to be installed to prevent access to the space underneath the deck. There was access to gardening tools by the deck, access to paint cans and painting tools, and access to shovels.
Emergency preparedness plan is not complete.
There are two cats on the premises. Rabies records are expired and need verification of updated rabies.

A secured gate needs to be installed at the top of the stairs leading to the basement door, which is accessible from the backyard.
