At the time of the visit the following was noted: 1. A wooden board to the edge of the roofing in the covered outdoor play space was missing. 2. A light was hanging off of the building and not secured, wires exposed.

At the time of the visit the following was noted: 1. Black mold/mildew was observed in the caulking around sinks in several classrooms and group bathrooms.2. The seams in the wall-to-wall carpeting were starting to fray in the lobby and gym areas posing a potential tripping hazard. 3. A floor tile in front of a toilet, in one of the children's bathrooms is broken.
When observing the outdoor play space, the following was noted: 1. The plastic coating on the stairs/platform to the Toddler climber is worn off and they are rusted. 2. The top of the chains to the Toddler swings were rusted.
