4 children's records were randomly selected for review, and the following was learned: 1) One child had a face sheet, first aid and emergency consent, transportation plan and developmental history form that were not signed or date by their parent as required. 2) Additionally, the same child did not have evidence of having provided the program with a written physical and MMR records as required.
During a review of the outdoor licensed space, the following was learned: 1) On the toddler playground with the climber, the poured surfacing is pulling up and has holes in several places, creating a potential hazard 2) bolts from pulling up from the railroad ties on the preschool playground, creating a potential hazard 3) The gate that sections off the back of the shed from the preschool playground has a door that no longer closes or secures, creating a potential hazard 4) the gate door that leads to the grassy back area (next to the gate to the toddler playground) is not fully secured, creating a potential hazard to children.

Three staff records were randomly selected for review and the following was learned: 1) Two educators did not have evidence of a staff information form as required 2) Two educators did not have written evidence of proving the program with a physical within one year when they were hired (4/20/24 and 9/24/2024) 3) Two educators did not have documented evidence of 2 MMR as required 4) Two Staff did not have written evidence of completed program orientation as required by regulation 5) One educators did not have documented evidence of a current EEC registry as required 6) One educators (start date 4/20/24) id did have documented evidence of completing the "EEC Essentials" 12 hours training as required 7) one educator (start date 9/24/24) has been allowed to work in the program prior to completing their "EEC Essentials" 12 hours of training, as required for all new to the field educators as of May 2024.

During a health and safety review of the program, the following was observed: 1) the toilet was broken in the preschool bathroom 2) There was chipping paint in the preschool classroom, creating a potential hazard to children 3) there were torn rest mats, with the foam pulling out, exposed to children in the Toddler room, creating a potential hazard 4) the ceiling vents were observed to be dirty and full of dust, potential creating an hazard to children.

Child B did not have evidence of a current IHCP plan as required by regulation (expired 11/23/22).
During a review of the indoor space, the following was learned: 1) toilet paper was stored in an unsanitary manner when it was stored on top of the broken toilet, rather than in the dispensers as required 2) Stained ceiling tiles were observed in "toddler C" creating a potential hazard 3) Adult scissors were observed to be stored next to the diaper table in "toddler C" creating potential hazard 4) The sink the children's bathroom on the preschool side was observed to be discolored and "rusty" looking 5) Not all road rugs were secured to the floor, creating a potential tripping hazard 5) the laundry room door was unsecured, creating a potential hazard 6) Not all children's feet touched the floor in the "Toddler C" room while eating, creating a potential choking hazard 7) In the "toddler D" classroom it was observed that the window screen had a big hole in it and the educator had opened that window, creating a potential hazard to children 8) The white vent in the "Infant C" room was observed to be dirty and full of dust.

2 Toddler rooms were observed to have paint chipping around the window ledges, creating a potential hazard.
During a review of the outdoor space, the following was learned: 1) on the toddler playground (on both sides) identified weeds were observed to be growing through the fence, creating a potential hazard. 2) the hose posed a tripping hazard when it was set up "over" the fence 3) Although the Director reported they had submitted a "work order" there was no evidence of a shade structure on the second toddler playground, where the children were observed to be using the sprinkler, as required by regulation. 4) Emergency backpacks, on both the toddler and preschool playgrounds, were observed to be stored on the ground, in the reach of children, posing a potential hazard. 5) the bolts were observed to be pulling up from the barriers on the preschool playground, creating a potential danger. 5) the gate that leads to the parking lot on the preschool side has a potential entrapment hazard between the pole and the fence gate, creating a potential hazard. 6) the metal pole that hold the gate together on the "covered section" of the playground was observed to be unstable, creating a potential hazard to children.
The sink space in the preschool children's bathroom in the hallway was observed to rusting inside, creating a hazard. In the bathroom next door, there was not toilet paper available to children.

During a review of the outdoor space, it was observed that on the PS playground, some of the nails are pulling up out of the rail road ties, creating a hazard. The fence that surrounds a heating/cooling unit on the toddler playground was observed to be rusting, creating a potential hazard. On that same playground space, the turf section is pulling up and has created uneven ground, creating a potential hazard to children.

1) Not all electrical outlets had covers as required 2) A cabinet with educator's purse stored inside (near the kitchen) was not secured as required 3) the cabinet in the Pre-K room was broken and could not be secured as required 4) adult scissors are stored in the reach of children in preschool D, creating a hazard.

8 children's records were randomly selected for review and the following was learned: 1) One child did not have documented evidence of a current face sheet as required 2) Two children did not have evidence of a current first aid/emergency consent form as required 3) Six children did not have evidence of a current transportation plan as required 4) Two children did not have their date of admission documented as required 5) five children did not have current developmental history forms as required 6) three children did not have current documented physical exams on records as required 7) two children did not have evidence of MMR on record as required.
Not all equipment, furniture, dramatic play set and cords were secured a required, creating a potential danger to children.
During a review of the outdoor space, the following was learned: 1) There is a fence that doesn't latch on the toddler side, creating a potential hazard 2) Additionally a pipe has fallen down near the gate, creating a hazard. 3) the bolts are pulling up from the railroad ties on the preschool playground, creating a potential hazard.
Toilet paper was not stored in a safe and sanitary manner when it was stored on top of the dispenser.
1) Educator A was observed to wipe the nose of Child A but did not wash their hands or the child's hands as required 2) Educator B conducted a diaper change but did not wash the hands of the child as required.
During a review of several classrooms, it was learned that the changing table was being used as a table to store equipment or the attendance clipboard, creating a potential cross contamination hazard.
In more than one classroom and bathroom space, it was observed that diapers are being disposed of in an unsealed container, creating a potential hazard.
Paint was observed to be chipping on the window ledge in "infant3" creating a hazard.

Three Children's records were randomly selected for review and the following was learned: 1) Child A did not have a current face sheet, developmental history, first aid emergency consent form or transportation form as required by regulation.
Educators were not observed to wash their hands prior to serving lunch in the "Pre-K" Room.
During a review of the outdoor space, the following was learned: 1) Bolts were observed to be pulling up from the railroad ties, creating a potential hazard on the preschool playground. 2) the climber did not have adequate woodchips surrounding the equipment as required on the preschool playground 3) On the main toddler playground, there was a broken plastic shed, with the pieces scattered around it, creating a potential hazard 4) the second toddler playground did not have evidence of a sufficient shade structure as required by regulation.
During a review of program medication, the following was learned: 1) Child B had an [REDACTED] that expired 11/23/22. 2) Child C was learned to be 4 years old but had [REDACTED] that state on the box "for use with children 6 years and older." 3) Child D had a [REDACTED] that expired
During an observation in the "Pre-K" room, the following was observed 1) children were observed to have their fingers in their noses while speaking to their educators, but they were not asked to wash their hands as required by regulation 2) Educators did not wash or sanitize the table, where children had been observed using table activities, prior to serving lunch as required.
During a review of a diaper change in the "infant/toddler" classroom it was observed that Educator A did not keep their hand on Child A while changing on a raised surface as required by regulation.
While the emergency medication was available for use in the "pre-K" room it was also observed to be stored in the reach of children, creating a potential hazard.

2 staff records were randomly selected for review and the following was learned: Educator A (start date 9/14/2021) did not have evidence of providing the program a physical within one year as required by regulation. The same educator did not have evidence of [REDACTED] on record at the program.
Three children's records were randomly selected for review and the following was learned: 1 child did not have evidence of current enrollment paperwork, including a face sheet, emergency medical consent form, a transportation form or a developmental history form as reviewed by regulation.
1) On the day of the visit, educator's bags and purses were observed to be stored in the reach of children or in cabinets/closets that were not secured, creating a potential hazard to children. 2) food scraps were observed to be thrown in a trash can without a lid as required in preschool 3) the sunscreen bottles were no longer labelled for individual use in "Pre-K" when the labels were found to be "rubbed off" 4) a toy shelf was found to be unsecured, creating a hazard in the toddler room.
During an observation of the cleaning practices in the "pre-K" room, it was observed that the educator did not use a two step cleaning and sanitizing process as required by regulation when cleaning the classroom tables.
It was observed that in two of the toddler rooms, chipping paint was exposed to children surrounding the window sills, creating a potential hazard.
