During a review of the outdoor play space, the following was learned: There were potential entrapment hazards founds on 2 gate doors and under the gate that divides the two playgrounds.
Three IHCP plans were found to be missing the following: One IHCP did not have evidence of a parent signature. One IHCP did not have evidence of the doctor signature, the treatment plan or potential side effects. One plan did not have evidence of the doctors signature, potential side effects, consequences ces if not administrated, who was trained in the child’s plan and who trained the educators.
