One educators file was missing the following information: hiring information, medical information, orientation, etc.
Educator not wearing gloves while peeling child’s banana and placing it on table.
Preschool classroom had a lot of clutter on top of cubbies and shelves. Minimize cluttered areas.
Upon arrival toddlers were utilizing playground with Pea Gravel as a ground cover. There is not sufficient gravel in fall zones. Gravel measured approx. 2". Sand measures approximately 2" under swings. Swings clearance to fence is approximately 4.5', clearance should be twice as much. Distance between swing structures not adequate.
Emergency maps and procedures must be posted at each exit. Maps need to highlight primary and secondary routes.
During the review of files it was learned that all children who have an allergy were missing Individual Health Care Plans, physicals were expired and one physical was missing a doctors signature.
One teacher was alone with a group of 8 toddlers at the time of the visit.
At the time of the visit the classrooms had safety concerns; there was a flat screen TV in a room that was not secured, an educators purse within reach of children and mirrors that were glass.
All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served. There was only one person who had current first aid at the time of the visit and she is not on location the entire day.

The licensee must ensure that at least one educator currently certified in first aid and age-appropriate cardiopulmonary resuscitation (CPR) is present at any and all times when children are in care.

During the virtual visit an epi pen did not have a prescription label affixed to it.

Toxic Substances
Attendance was not accurate at the time of the visit, in the toddler room none of the children were signed in.
In the gym there was chipping paint, the bathrooms had toilet brushes and plungers that were accessible to the children. There was a TV in the gym that was very easy to move. The cube shelf's in the preschool room were wobbly.
At the time of the visit toxic substances and hazards were accessible to the child. In the toddler room there was a glade spray within reach of the changing table, in the preschool and pre-k classroom hot coffee was within reach of the children, several of the outlets were missing outlet covers in the classrooms, gym and nap room. There was an educators purse in the preschool room that was accessible to the children.
