One child's medication was stored in a ziploc bag and was not stored in the original packaging with the label affixed.
One child's IHCP was not complete and did not contain information as required by EEC.
One child had 4 medications; there was evidence of a medication consent form for only one of the four medications. Also, at least one child's medication was expired.

Program reports that they use text messaging and group chats for staff meetings. There is no evidence to document who attended staff meetings, the dates, and the agenda.
During the visit, 30 children are watching a movie in a room with a capacity for less than 30 children. The children utilized the room to watch a movie for over an hour during the renewal visit.
At least one IHCP was expired during the visit. IHCP's must be updated annually to reflect child current medical needs.

Two of the three files did not have Strong start essential trainings. The educators have been employed longer than six months.
Equipment is present, but the equipment did not appear to be available to the students. At the time of the visit students were not offered opportunities to utilize the equipment or other learning activities.

Educator alone with 16 children, playing games on the mat. The other educator is in the front of the building answering the phone, and checking children in as they arrive on the bus. Educator was over ratio for at least 15 min. Group of three girls were sent to wash hands upon arriving. The bathroom is on the back side of building. Educator’s were in the front of the building and did not monitor children in the bathroom.
All children were instructed to play Duck, Duck, Goose on the mat along with other relay type running games. No other learning options or activities were provided to children during the visit.

Emergency medications readily available
During the visit, it was observed that controlled substances are not stored in a secured and locked place.
On the day of the visit, a review of the attendance records for several days during the month of October 2019 revealed that for most of the days reviewed, the Program had in attendance more than the licensed capacity allowed.
