In the Infant/Toddler classroom, a play kitchen set was not secured to prevent it from tipping.
The following was observed outdoors: there was not six feet between the large climbing structure and the see-saw on the preschool playground. There was only four feet between the brown climber and large climbing structure on the toddler playground.
This investigation inspection recorded no violations or advisories.
The fall zones for many of the climbing structures located throughout the playground did not have a minimum of 6 feet in all directions from the perimeter of the equipment.

In the Pre-K bathroom and rear hallway, shelving with materials and supplies were accessible to the children. The materials and supplies were not secured and posed a hazard to the children. In the Toddler 2 classroom, one ceiling tile was stained and another had a hole in it.
The following hazards were observed on the playground: unknown vegetation was growing through the chain link fence in many areas. There were two umbrella stands not in use, leaning against the fence and posed a hazard to the children.
