At the time of the visit staff records were reviewed and the following was found: one staff did not have evidence of a current physical or MMR in their file, one staff had an expired physical, and one staff's file did not contain evidence of MMR.
At the time of the visit three children with emergency medical conditions were reviewed and the following was found: one child was missing a portion of their prescribed medications that are required to be onsite and accessible at all times.
