Outdoor Space - Square Footage
Upon arrival, the licensor observed that the nap room door was fully closed, with two children resting on mats inside. When this was brought to the educator’s attention, she stated that she had cameras; however, when the licensor viewed the footage, visibility was limited due to poor lighting and the camera’s positioning.
During the visit, it was observed that two kitchen play sets were not attached to the wall, posing a tipping hazard.
When the licensor arrived the door to the nap room was shut tight and 2 children were on mats behind the closed door

A review of 4 records show that: one child needs authorized pick up person, date on parent handbook and parental visit statements, and an update physical. Two children need the general permission completed.
The nap mats were not spaced 2ft apart
There is only 1 surface for diapering
The educator has a slide that is above 18 inches and requires ground surface covering
Outdoor Playspace - Hazards
The educator did not wash the child’s hands after diapering.
The first aid kit was missing the emergency contact information
The licensor observed the following in the TV room: there is a cord accessible to a lamp and the lamp is accessible and a tipping hazard, the brick fireplace is not covered around the edges

A review of 6 children’s records show that: 2 general permissions were blank, 1 date of admission was missing, 1 physical description was missing, 1 emergency contact was missing: 3 children were missing medical documentation, 1 medication permission was blank and 1 child was missing proof of a lead test
There are several children in care who required progress reports and they do not have them.
There are 3 children in care with identified allergies and no required postings
The licensor observed the following: several times that children went into the TV room and the Educator was not able to see them from the playroom. Additionally when she was in the TV room she was not able to see the children in the playroom. One child was observed going down the hall into unlicensed space and one child was observed pulling out the heating vent on the floor and putting his hands and toys in the vent.
The fence that was installed was not installed all the way around, there is one area that needs secured so that the children cannot access the street
The Educator did sit the children down for a story today and she reviewed the day and the date with the children, she also sat on the floor and engaged in blocks with them. The children were not engaged in these activities. The Educator needs to structure the day for the younger children in care.
