The licensor observed 1 window screen in one of the playrooms to have a hole in it and the window was open from the bottom
Indoors: the cubbies were not secure to the wall; kitchen: 3 drawers were not locked and have hazards accessible including but not limited to: sharp knives, peelers, scissors, lighter Outdoor: the children were observed playing in the driveway. The driveway does not have a 4ft fence preventing the children from accessing the busy street. Outdoor space cannot be approved without fencing

The Licensor observed that the diapering table is not bolted and could pose a tipping hazard. The Licensor observed that the bolt to the cubbie is loose and could pose a tipping hazard. The tall doll house is not bolted and could pose a tipping hazard. Two shelves in the basement level are not bolted and could pose a tipping hazard.

Two electric baseboard in the basement level have exposed sharp grates.
There were drawers in the kitchen that had hazards in them accessible children (knives, sharp objects, etc.). There were shelves in the two playrooms that were tipping hazards. The changing table was not secured to the wall and was a tipping hazard. There were electrical cords hanging loosely in the living room/dining room area that were strangulation hazards. The trash in the bathroom did not have a cover and was accessible to the children.
