During the visit, two staff files were reviewed. One file did not include documentation showing the completion of the Essentials 2.0 trainings. The second file did not include the following: Staff Form, resume/application, verification of references, current PQ Registry, evidence of immunity to Measles, Mumps and Rubella. Program Orientation. Observations of both educators working with children were not available for review.
The two transportation drivers have not completed the Essentials 2.0 - Transporting Children Safely Training.

During the visit three children's files were reviewed. Once file did not include a lead test date.
During the visit, the program's fire drill logs were reviewed. June 2, 2023, was the last documented practice drill for the toddler and preschool age groups. March 8, 2023, was the last documented practice drill for the school age children. Practice evacuation drills must be conducted monthly and documented in the Evacuation Drill Log.

An Individual Health Care Plan was incomplete. Consequences of not giving the medication was blank. Side effects of the medication was blank.The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
