Current consent for administered medications
1) Child A had [REDACTED] on site that expired 6/2025 2) Child A had a an IHCP that wasn't clear, the plan stated to give Child A [REDACTED] but did not reference the [REDACTED] they had on site/consent for as required 2) Child B had "infant/toddler[REDACTED]that expired 10/25, additionally though the box states this medication can be given until the child turns 3, the medication consent form stated [REDACTED] (which means the dosage on the form is different than what the instructions on the box require) 3) Child B had an [REDACTED] on site that expired 12/2025.

Two staff records were randomly selected for review and the following was learned: 1) Educator A did not have written evidence of a physical exam on record as required 2) Educator B (start date 9/9/25) did not have evidence of having provided the program a physical exam within one year or[REDACTED]records as required by regulation.
During a review of the program's indoor licensed space, the following was observed: 1) Chipping paint and chipping wallboard in the "preschool" classroom, creating potential hazard 2) the heating cover in the children's bathroom was observed to be "rusty" creating a potential hazard to children 3) In the "toddler" classroom, the educators purses, drinks and cell phones were stored in the reach of children, creating a potential hazard.
Child A had an IHCP form and a medication consent form that were not signed and dated by the child's parent as required by regulation.
In the "toddler room" it was observed that not all toy shelves were secured as required by regulation.
Electric outlets covered, no frayed electrical cords. Carbon monoxide detector.

One outlet cover was missing in the preschool room, creating a potential hazard.
On the day of the visit, the staff schedule was not posted as required and the location of the health care policy was not posted as required.
This provisional license 90-day monitoring visit inspection recorded no violations or advisories.
During a review of 2 diaper changes in the "toddler room" the following was observed: 1) Educator A did not keep a hand on the child while changing on a raised surface as required 2) Although Educator A did change the gloves they were wearing between diaper changes, they did not wash their hands with running water between diaper changes as required by regulation 3) Educator A did not have Child A or Child B wash their hands after changing their diaper, as required by regulation.
