Child A - The prescribed emergency medication was not in the container in which it was originally dispensed and with its original label affixed. There was no expiration date listed on the medication.
Child A - the educator did not have one of the emergency medications listed on the child's Individual Health Care Plan on site.

Ten children enrolled, nine present at the time of the visit. Six under two years old including two infants. Certified Assistant for 6 Kenia Coelho and Regular Assistant Debora Pereira Costa were caring for the children. The educator stated that she was on the premises but the licensor waited about 25 minutes to see the educator after the CA texted her informing that the licensor was in the home. **The educator was not present in the daycare space and none of the two assistants are licensed for 10 children. The assistants were operating above their capacity at that time. **

There was a climber set with slide and ladders on grass with no fall zone installed. One slide that is higher than 18 inches on grass. Please remove the equipment or install a fall zone. Emailed the Playground Safety Handbook.
Art room: The front loading washing machine is missing a safety lock. Home heating equipment accessible, only separated from licensed space by a curtain. Please install a barricade to prevent access.
