Child C - 1) The most recent medical form lists three food allergies, but the Individual Health Care Plan completed by the parent only lists one. 2) The medical form lists a second chronic health condition requiring medication and there is no IHCP and medication for this condition on site. Please have the child's doctor complete a new IHCP listing all allergies and medical conditions and respective medications.
