Emergency medication was stored in the reach of children when it was hung up inside the "short side" of the fence on the preschool playground.
During a review of the indoor licensed space, the following was observed: 1) In several classrooms and children's bathroom spaces, the "kick board" surrounding the base of the wall was observed to have pulled away from the wall, creating a potential hazard to children 2) in the children's bathroom in "room 6" one of the children's sinks is "rusty" and the plaster is chipping off, creating a potential danger to children 3) several classrooms were observed to have chipping paint, creating a potential hazard to children 4) In "room 9" the toy shelf is no longer secured, with chipping wall board and the screws pulled out of the wall, exposed to children and creating a potential hazard.

6 children were signed into the infant room, but 7 children were present.
During a review of the outdoor licensed space, the following was observed: 1) Unidentified weeds were observed to be growing over the fence and through it, creating a potential hazard to children 2) The "blue child size table" was observed to be behind the climber, within the 6 feet of required fall zone, creating a potential hazard to children 3) Educator A's "iced tea" was stored in the reach of children when it was left on the small table by the climber on the preschool playground, creating a potential hazard.
Chipping paint, and holes in the wall exposing wall board were observed, creating a potential hazard to children.

Although the program recently started their new school year, moving classrooms and furniture, the following was observed: 1) not all toy shelves or dramatic play sets were secured, creating a hazard 2) Additionally, stained ceiling tiles were observed throughout the program, creating a potential hazard to children.
Unidentified weeds and branches were observed to be growing over and through the fences, creating a potential hazard to children.
This technical assistance visit inspection recorded no violations or advisories.

Three Children's records were randomly selected for review and the following was learned: 1) Child A did not have evidence of a current physical,[REDACTED]records or lead screening as required by regulation.
This investigation inspection recorded no violations or advisories.
During a review of all program IHCP and medication, the following was learned: 1) Child A has an IHCP that requires [REDACTED] to be on site as part of their emergency medications. On the day of the visit, Child A did not have "Benadryl" on site as required.
Unidentified weeds were observed to be growing through the playground fence, creating a potential risk to children.

4 children's records were randomly selected for review and the following was learned: One child did not have evidence of a written developmental history form as required.
During a review of the program's indoor space, the following was observed: 1) In the "preschool" group bathrooms, the toilet paper was observed to be stored in an "unsanitary" manner when it was left on top of the dispenser, rather than in the dispenser for individual use. 2) Paper towels were not accessible or available for individual use in the same bathroom space as required 3) one of the ceiling tiles was observed to be stained in the same bathroom, creating a potential hazard to children 4) the caulking behind the sink in the same bathroom was observed to be coming loose, and discolored, creating potential risk to harm to children.

During a review of 2 randomly selected staff records, the following was observed: 1) Educator A was being used in a "teacher certified" capacity, but during the record review it was learned that the program did not have documentation of their work experience versified as required. This was addressed with the program during the April 2023 investigation visit. 2)Educator A did not have documentation of a physical within one year as required. 3) Educator B did not have 2 documented professional references as required
Three children's records were randomly selected for review and the following was learned: 1) Child A did not have enrollment paperwork under "Cadence Academy" as required.
During a health and safety assessment of the program, the following was learned: 1) In "room 4" the diaper table and other toy shelves were not secured as required 2) in "room 4" and in the "preschool bathroom" the caulking around the children's bathroom sink is discolored and rusting, creating a potential hazard 3) In "room 4" the Kickboard next to the changing table is pulling off of the wall, creating a hazard 5) In "room 1" food scraps were observed to be thrown into a trash can without a lid.
During a review of the outdoor door space, the following was learned: 1) There is a gap between the wooden fence and the metal post, creating a potential hazard (on the gate separating the two playgrounds) 2) The gate to the toddler playground is not secured, as the latch does not properly lock, allowing children potential access to the parking lot 3) unidentified weeds were observed to be growing over and under the wooden fence, creating a potential hazard.
