During a review of the program's indoor space, the following was observed: 1) Not all cords were secured, creating a potential hazard 2) toilet paper was observed to be stored in an unsanitary manner when it was placed on top of the toilet tank, rather than on the toilet paper dispenser in "room 10" 3) There was no toilet paper available to children in preschool "room 11"

During a health and safety review of the program, the following was observed: 1) In "Room 11-Pre-K" Toilet paper was stored in an any unsanitary manner when it was stored on top of the toilet paper dispenser 2) Diapers were thrown into a container that was not sealed in "room 11-Pre-k" as required by regulation.
During a review of the playground space, the following was observed: 1) the hose was left on the ground, creating a potential tripping hazard.
The toileting policy was not posted in the "room 10" as required by regulation.
5 staff records were randomly selected for review and the following was learned: 1) Two staff did not have evidence of current fingerprints on record as required.

During a review of the program's environment, the following was observed: 1) Stained ceiling tiles throughout the program, creating a potential hazard to children 2) The Changing table in "room 6" was found to be unsecured, creating a potential danger.

14 Children's Records were randomly selected for review and the following was learned: 1) 3 children did not have evidence of signed and dated paperwork under the new program umbrella "Cadence" as required by regulation 2) 9 children did not have evidence of their recorded date of admission as required by regulation 3) One child did not have evidence of having provided the program with a current physical as required by regulation.
On the day of the visit, a current staff schedule was not posted where parents, visitors and staff can review as required by regulation.
Children were not observed to wash their hands before and after water play as required by regulation.
2 children's IHCP plans were reviewed, and it was learned that Child A had an IHCP plan that expired 3/4/2023 and a medication consent form that expired 2/28/2023.Child B had an IHCP plan that expired 3/18/2023 and medication consent forms that expired 3/18/2023.

2 staff records were randomly selected for review and the following was learned: 1) Educator A did not have evidence of a current physical on file. 2) Educator B did not have evidence of proving the program 2 [REDACTED] as required and did not have evidence of additional professional development hours as required.
4 children's records were randomly selected for review, and it learned that one child had not signed paperwork since 2021. It was also learned that during the recent sale of the program, families were not registered under the new umbrella company as required.
During an observation in "Room 6" it was observed that Educator A physically redirected children, rather than speaking with them. This same educator was observed to bend over and state in front of a toddler "no" more than once.
During a health and safety assessment of three classrooms, the following was learned: 1) Room 6 had a shelf that was not secured, creating a potential hazard. 2) the supply closet in the gross motor room was not secured, creating a potential hazard
