All staff records were reviewed, and the following was found: 1) One staff member did not have evidence of a current physical.
During the visit, the attendance record was inaccurate. One child was dropped off, but the educators did not sign them in. There were 15 children signed in, while 16 children were actually present.

During the inspection of the sunset classroom, the following was found: 1) The circle rug was not flat, causing a tripping hazard. 2) In multiple areas, the hardwood floors were separating and lifting.
Supervision of Children
Three staff files were reviewed, and the following was found: 1) Three staff members did not have evidence of current PQ Registry. 2)
At the time of the visit, an Educator in the Sunset classroom did not position themselves to maximize their ability to see and/or hear children in their care. A child left the classroom, entered the Director's office unsupervised, and requested to use the restroom.
Individual Health Care Plan (IHCP)
At the time of the visit, the fire inspection had expired.
Six staff records were selected to review, and the following was found: 1) Four staff members did not have evidence of a staff form. 2) Two staff members did not have evidence of documentation of an interview. 3) Three staff members did not have evidence of a current EEC PQ Registry. 4) Three staff members did not have evidence of [REDACTED]5) Five staff members did not have evidence of a completed program orientation. 6) Five staff members did not have evidence of bi-monthly observations. 7) One staff member did not have a staff evaluation.
During the IHCP review, the following was found: 1) The program did not obtain one child's required medication. 2) One child's paperwork did not have evidence of parent signatures.

During the Medication review, the following was found: Two children did not have evidence of medication consent for each medication on site.

Three staff members did not have completed BRCs at the time of the visit. Two staff members were able to complete their BRCs while the Licensors were at the program. One Staff member may not return to the program with their BRC completed.

During the visit it was learned a 14-year-old child was there to volunteer. The program does not have a volunteer policy/plan on file and the minimum age of a volunteer is 15 per EEC's Youth Worker's policy.
During the visit educators were observed sitting in chairs on the playground and only interacting/engaging with children that came over to them.
Program does not have a full time Director qualified administrator on the premises full time. Director is only on the premises periodically during the week.

Three staff records were randomly selected for review, and the following was learned: 1) One Staff needed evidence of a current physical required by regulation. 2) One Staff did not have evidence of a First Aid Certificate. 3) Two Staff did not have evidence of the Bi-monthly observations. 4) Two Staff did not have evidence of staff evaluations.
The paper attendance in the Sunset Classroom was not accurate. There were nine children signed in, and ten were present.

At the time of the visit, multiple shelves in the Sunset Classroom were not secured.
At the time of the visit, not all emergency exits had the appropriate posting.
At the time of the visit, the Sunrise Classrooms exit/evacuation route needed to be clear of obstructions. An AC unit and books were found in the way of the classroom's emergency exit.

The fencing to the outdoor play area was weathered and splintering. The sandbox was splitering and exposing nails.
In reviewing staff records, the following was noted: 1. A staff record did not have evidence of any references called nor the date of interviews. 2. The wasn’t a date of a staff’s date of physical.

Sunscreen spray and lotion were accessible to children on the playground causing a potential hazard.

Parts of the fencing was not secured; creating a potential hazards.

At the time of the visit, the following was observed: One Educator from the Sunset Classroom did not maintain accurate staff-to-child ratios. An Educator left the classroom with one child, leaving the other Educator alone with 16 children.
In the women's bathroom, the program did not post its written plan for diapering and toilet training in the diapering areas.

Three children's records were selected to review. The following was found: 1) Two children did not have evidence of a date of admission. 2) Three children did not have evidence of a progress report. 3) One child did not have evidence of a current Physical.
At the time of the visit, the following was observed: 1) The Sunset Classroom did not maintain accurate attendance. Twelve children were signed in, and fourteen children were present. 2) One educator from the Sunset Classroom did not maintain accurate staff-to-child ratios. When the Licensor arrived at the program, one educator was seen walking off the playground into the program, leaving the other Educator alone on the playground with fourteen children.
Three Staff Records were randomly selected to review, and the following were found: 1) Three staff members did not have evidence of a staff records form. 2) One staff member did not have evidence of a current physical. 3) Two staff members did not have evidence of a completed EEC Essentials Certification 4) One Staff needed evidence of 2 doses of [REDACTED] as required by regulation. 5) Three staff did not have evidence of completed staff orientation forms.
During the Children's Records review, the following was found: 1) One child did not have evidence of a current Physical Exam. 2) One child did not have evidence of a date of admission.
During the IHCP and Medication Review, the following was found: 1) Two children's IHCP/ Medication Paperwork expired. 2) One Child's Medical Consent forms were missing parent/guardian signatures.
Educators were not appropriately supervising children on the playground. Educators sat in chairs in one corner and most of the children were playing in the dramatic playhouse on the other side of the climbing structure, which significantly limited their ability to see and/or hear all the children.
During the Safety inspection, the following was found: Multiple Shelves throughout the program needed to be secured.

There must be a barrier between children’s space and the kitchen.
In reviewing children’s records, there wasn't evidence of parental consent for sunscreen that was being applied on the children's skin.

Cords to window blinds were hanging within children's reach, causing a potential hazard.
The door between the children’s activity space and the kitchen was open allowing children access to the kitchen area where educators store their personal belongings.
