Three children's records were selected to review, and the following was found: 1) Two children's records were found to be out of date.
During the health and safety review, the following was found: Peeling/chipping paint was found in all classrooms of the program.
During the health and safety inspection of the program, the following was found: 1) One Educator in the Venus classroom did not wash their hands before handling food. 2) Educators in the Venus classroom did not wash and disinfect the tables before use. 3) The Educator in the Saturn Classroom did not wash and disinfect the tables after use.
In the Saturn Classroom, an educator's bag was found accessible to children.

During a diaper change, the following was observed: 1) One Educator did not always keep at least one hand on the child when the child was being changed on an elevated surface. 2) One Educator did not wash the child's hands with liquid soap and running water using friction and dry their hands with individual or disposable towels after diapering a child. 3) The educator then used the food preparation sink to wash their hands after completing the diaper change when the program was warming bottles.

Four staff records were randomly selected for review, and the following was learned: 1) One staff member did not have evidence of interview documentation. 2) One staff member did not have evidence of verification of two references. 3) One staff member did not have evidence of a staff form, application/resume, interview documentation, and verification of two references.
This provisional license renewal visit inspection recorded no violations or advisories.

At the time of the visit, snacks in the Infant/Toddler and Preschool Classrooms were not stored in clean/covered containers.
During the safety and health review of the program, the following was found: 1) Multiple shelves, bookshelves, and dramatic play equipment needed to be secured. 2) An outlet cover was missing in the Preschool Classroom. 3) A Swiffer wet jet was found within children's reach in the Preschool Bathroom.
During the safety review of the program's outdoor space, the following was found: exposed landscaping fabric sheets under the mulch barriers were causing a tripping hazard.

During the safety check of the program, the following was found: 1) The cubby shelf in the preschool room was not secured down. 2) The blind strings in the gross motor room were dangling down in children's reach.
During the view of Staff Records, the following was found: 1) Two staff members did not have evidence of Documentation of Interview, Resumes/ Application and Verification of 2 References. 2) One staff member did not have evidence of [REDACTED]
At the time of the visit, the program did not have a thermometer in the refrigerator.
Six children's records were randomly selected for review, and the following was learned: 1) Two Children's paperwork had expired. 2) Two Children did not have evidence of medication consent forms.
During the safety inspection, the following was observed: 1) In all three Classrooms, chipping/peeling paint was found. 2) In the Venus Classroom, a cord was plugged in and hanging off the counter within reach of the children.

Three children's records were randomly selected, and the following was found: 1) Two children's files needed evidence of a transportation plan. 2) One child's file did not have evidence of the medical consent forms required by regulations.
Four staff records were randomly selected, and the following were found: 1) One staff member needed evidence of application/ resume, verification of two references, current PQ registry, Physical, and [REDACTED] 2) Two staff members needed evidence of documentation of the interview.
At the time of the visit, The Infant/Toddler Classroom did not maintain accurate staff-child ratios when one educator left the classroom, leaving the other Educator with five children.
At the time of the visit, the diapering and food preparation/service area in the Infant/Toddler Classroom were not separate, as the high chairs were up against the back of the diaper-changing table.
At the time of the visit, not all children's water and milk bottles were labeled in the Infant/Toddler Classroom.
During the Medication review in the Infant/Toddler Classroom, the following was found: one child's medication expired on 11/23.
