Staff records were selected for review, and the following was found: 1) Three Educators did not have evidence of documentation of two references. 2) One educator did not have evidence of a current EEC PQ registry.
During the visit, the educators in Toddler A did not maintain appropriate staff-to-child ratios: one educator left the classroom, leaving another educator alone with eight children.

During the health and safety review, the following was found: A ceiling tile in the Toddler A Classroom was hanging down.
The second evacuation route in the Toddler A Classroom was not kept clear of obstructions.
This investigation inspection recorded no violations or advisories.

Three Staff Records were selected for review, and the following was found: 1) Two staff member did not have evidence of completed program orientation.
During the programs Medication/IHCP Review, the following was found: 1) One child in the toddler classroom did not have evidence of a required medication listed on the allergy action plan. 2) One child's allergy action plan was missing their guardians signatures.
This change of space visit inspection recorded no violations or advisories.

The attendance in the Preschool Classroom was not accurate. Nine children were signed in, and ten children were present.
At the time of the visit, written emergency/ evacuation procedures were not posted to each exit in the Toddler and Preschool Classrooms.
At the time of the visit, a purple lighter was found in the plants in reach of children in the Preschool Classroom.
