At the time of the visit, one educator's backpack was found accessible to the children in the Toddler/Preschool classroom.
Four staff records were selected to review, the following was found: 1) One staff member did not have evidence of a date of hire. 2) One staff member did not have evidence of a current PQ registry. 3) One staff member did not have evidence of their MMR. 4) Three staff members were missing bi-monthly observations.
Four children's records were selected for review, the following was found: One child did not have evidence of a current physical.

The written plan for diapering and toilet training was not posted in the Toddler/ Preschool bathroom.
At the time of the visit, the following was found: 1) The white paint of the wooden playground fence was chipping, peeling, and flaking in multiple areas. 2) A piece of the mulch bedding was sticking out, causing a tripping hazard for children.
At the time of the visit, an Educator in the Toddler/Preschool Classroom did not use the two-step cleaning process when cleaning the table. The Educator only used the sanitizing/disinfecting spray.
Two Staff Records were randomly selected, and the following was found: 1) One staff member did not have evidence of a current physical. 2) One staff member did not have evidence of an application/resume.
During the safety inspection, the following was observed: Chipping/peeling paint was found throughout the Pre-K and Toddlers' classrooms.
Indoor Space
At the time of the visit, The Toddler Classroom did not maintain accurate staff-child ratios when one educator left the classroom and went into the kitchen, leaving the assistant educator alone with five toddlers for towels requested by a preschool school in the other classroom.

Three staff records were randomly selected for review, and the following was learned: 1) One Staff needed evidence of a completed program orientation and interview date. 2) One Staff needed evidence of the EEC Essentials Certificate required by regulation.
During the safety inspection of the program, the following was found: 1) Chipping and peeling paint was found in all three classrooms. 2) Shelves and dramatic play equipment must be secured in the Preschool and Pre-K classrooms. 3) Bathroom Trash Containers were not covered.
Evacuation procedures posted at each exit
The plastic roof to one of the playhouses was cracked, exposing sharp edges. The fencing in the far back of the playgroup was not free of entrapment hazards.

For under 3, guards at top/bottom of stairwells
During the visit, there was a pressure gate at the top of the stairs near the preschool cubbies. Pressure gates may not be used at the top of stairs. The barriers must be permanently installed at the top of stairways. The [REDACTED] will remove the pressure gate and install a permanent barrier.
