Licensor inspected all of the licensed, indoor space during the renewal visit, as well as the outdoor playspace (fenced in back yard). The following hazards were observed: ** KITCHEN **- top drawer and middle drawer on the far left side of the kitchen both have hazards inside ** 2 BEDROOMS **- no hazards ** Wants as to add 3rd bedroom** - book shelves are a tipping hazard, outlets exposed ** PLAYROOM **- no hazards ** BATHROOM ** - hygiene products in the shower that were lower than 4ft. ** OUTSIDE **- still using swings on the play set with no fallzone.
Licensor randomly reviewed 4 of the children's progress reports. Of these, 2 were complete and up to date, but the other 2 were out of date.

Educator stated she has conducted a fire drill every month. However. Educator could not locate the evacuation log during the renewal visit.
Educator's adult son recently moved home about a month ago. However, he has not been added as a household member.
For diapering, Educator uses a roll of parchment paper on the carpet in the playroom. Licensor discussed with Educator that diapering needs to be done on a surface that can be easily disinfected, and then a disposable layer needs to be used on top of that surface.
Educator has been using all 3 bedrooms in the home, but only 2 bedrooms are currently licensed.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN** - 2 drawers to the left of the stove have hazards inside (batteries, sharp knives, chokable items) and need to be made inaccessible **2 BEDROOMS** - no hazards **PLAYROOM** - no hazards **BATHROOM** - no hazards **OUTSIDE** - no hazards, fenced in back yard.
All of the required items were observed in the first aid kit. However, the children's emergency contact forms need to be updated.

Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN - no hazards **2 BEDROOMS - in the master bedroom, there were small screws in top drawer of the nightstand on the right side of the bed and an outlet not covered **PLAYROOM - no hazards **BATHROOM - no hazards **OUTSIDE - Play structure is not used and the swings are removed. However, the slide and ladder are still present and access to them needs to be blocked.

Progress Reports

Licensor discussed with Educator the need for a list to be posted of all emergency or lifesaving medications, including but not limited to [REDACTED] that specifies to which child they belong. Licensor also discussed that this information needs to have a cover sheet in order to protect the child’s confidentiality.

There is a large play set with no fallzone as well as plastic slides above 18 inches. This playset was addressed at a previous visit and a fallzone has not been added.
The water temperature was 130F degrees. Regulations state the temp must be under 120F degrees.

Licensor reviewed 4 of the children's records. The following was observed: **1 - out of date signatures on the enrollment packet and out of date physical, no evidence of lead testing **2 - no medical information **3- no medical information **4 - out of date signatures and physical, no evidence of lead testing. Please review all of the children's records and ensure they are all complete and up to date.

Educator reported that she has not completed progress reports this year yet as the program began again in September. She stated she generally completes the progress reports in December and May/June. Last year, she gave the parents the progress reports and did not keep a copy in the children’s reports. Because of this, Licensor was not able to verify when the progress reports were last completed.
Licensor was able to inspect all of the licensed space in the home, as well as the outdoor playspace. In the home, the only room that had any hazards was the master bedroom. The nightstand drawers had hazards such as medications and small chokable items in them. Educator made it clear that she has not used this room for childcare in over a year. Outside, Educator utilizes the fenced in backyard as the outdoor playspace. Educator recently added a large play structure, which does not have a fall zone and therefor is a hazard.
Educator did not complete a fire drill in January or March. A fire drill was completed in February and in every month that Educator was open last year.

Educator reviewed one of the children's records during the virtual visit and it was complete and up to date, except for the progress report. Educator stated her plan is to complete progress reports on all of the children over April break in a few weeks.

Educator had 30+ training hours. Educator completed all trainings in 2019 and no trainings in 2017 or 2018. Please ensure 10 hours of training are completed every year.
First Aid Kit was missing gauze roll, cold pack, gloves, tape, and emergency cards. Educator must have a First Aid kit that includes the following: *CPR Mouth Guard, * Gauze Pads, * Gauze Rolls, * Tape, * Band-aids, *Disposable Ice pack, *Tweezers, *Scissors, *Thermometer, * Non-Latex Gloves, *Emergency Cards with each child's parental contact information.

There are several children who just started in September and are not due for a progress report. For the returning children, the last progress reports were completed in February so the new reports are a bit overdue.
All of the electrical outlets were covered except for one outlet in the back bedroom, which was broken and exposed.
Licensor inspected all of the licensed, indoor space during the renewal visit. The following hazards were observed: ** Pet food bowl in the hallway ** KITCHEN - Clorox cleaners on counter ** 2 BEDROOMS - back room had pet food on the floor and exposed outlet ** PLAYROOM - disinfectant wipes in the cabinet under the fish tank ** BATHROOM - Febreze spray and hygiene products on table, hair dryer plugged in to the wall and the cord was hanging down, hygiene products inside bathtub below 4 ft.
All of the required items were observed in the first aid kit, except for the children's emergency contact forms, which were either missing for the new children or out of date for the older children.
Educator did not have a copy of her Parent Handbook for Educator to look at during the renewal visit. She was able to email the EEC Parent Handbook to Licensor after the visit. However, none of Educator's program information was written into the handbook.
Diapering is done on the floor on a thin changing mat. Educator does not typically use a disposable layer, but does disinfect the mat after each use.
Only 2 of the children have been enrolled long enough to have a progress report, but there was no report for either of them.
Educator did not have an evacuation log. Educator stated she’s been doing fire drills but did not have anything documented.
CPR/1st Aid Expired 01/2022. Educator reported that she has a class scheduled for Monday (11/14).

There was a large metal frame with hooks that was used to hold a hammock, which is accessible to the children, as well as large lawn equipment.
Educator completed 28 hours of professional development in 2022. No hours were completed in 2021 or 2020. Regulations state that 10 hours of professional development are required per year, totaling 30 hours per licensing cycle.

Licensor reviewed seven records. Five were missing the annual update form. Two of the children's record were missing the basic transport signature. One child's record was missing an updated [REDACTED] Please ensure these records are updated and include a plan on how you maintain these records up to date in the future.
Licensor walked through licensed space and observed the following hazards: Indoor: **Kitchen** There was an unlocked junk drawer. There was a knife in an drawer that has a broken lock. There were cleaners, lighters and scissors on counter. Please make all these hazards inaccessible to childcare children. **Bathroom** There was a curling iron plugged in by the sink. There was mouth wash on top of toilet. There were personal hygiene products located in bathtub. Please ensure all of these items are made inaccessible to childcare children. **Bedroom 1** There is a TV that is not anchored. There are loose wires in one of the drawers. **Bedroom 2** There were junk drawer that need to be locked. **Bedroom 3** Educator's son was home sick and Licensor could not observe this bedroom. Licensor will complete a follow up visit to assess this bedroom. **Family Room** There were loose wires and dog food accessible. There were electrical outlets not covered. **Playroom** There was an uncovered trash can. Please ensure all of these hazards are resolved. Outdoor: There was chipping paint on shed door and rear door frame. The outdoor fence needs to be repaired.

Educator does not brush children's teeth and there were no oral non-participation forms on file for any of the children. Please ensure toothbrushing is done once a day, or that oral non-participation forms are signed by parents and filed for each child.
There was chipping paint located on the shed and rear door frame. Please ensure there is no chipping paint on the exterior or interior of the home.
Educator has been utilizing a third bedroom that is not on license. Please ensure only EEC approved space is being utilized for child care.
