In the downstairs preschool classroom, there was an unhooked door that lead to the kitchen area. There were hazards in the kitchen accessible to children.
In the preschool playground there were a couple of shingles that were broken and sharp on the edges. The shingles are hazardous and accessible to children.
On the bottom of the front door of the building there is some peeling and flaking paint accessible to children.

There was a small amount of peeling paint at children level in the toddler classroom near the climber.
A review of medications at the program indicated the following: -One medication expired on 9/19/23. -One child had a medication consent form and Individual Health Care Plan for medication however there was no medication available.

The middle bathroom in the downstairs classroom did not have posted a toileting/diapering plan.
A review of the individual health care plans indicated the following: -One did not have a current individual health care plan. -One did not have a current medical consent authorization form.

There was some unknown vegetation growing on the back fence of the preschool playground.

The new refrigerator did not have a thermometer.
A review of seven children's records indicated the following: -Two did not have a completed Individual Health Care Plan. -Seven did not have documentation of who trained the educators to administer the medication and which educators were trained to administer the medication.

Chipping and Peeling Paint
The main door into "The Toddler House" had white peeling paint. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
A review of the Emergency Evacuation Log indicated monthly required drills did not occur in 12/2019 and 2/2020.
This investigation inspection recorded no violations or advisories.
