A review of staff records indicated the following: -Three did not have documentation of an interview. -Two did not have documentation of two professional references. -One did not register annually with the Department of Early Education and Care. -Two did not have documentation of completing a physical examination. -Two did not have documentation of being immune to MMR. -Two did not have documentation of completing the program orientation before working with children. -One did not have documentation of having an annual evaluation.

A review of children's records indicated the following: -One did not have a non-expired (REDACTED) at the program. The (REDACTED) is an emergency medication.
One educator did not have written documentation of completing a First Aid Course as required. All educators must have current First Aid Training within 6 months of hire.
Two employees BRC's expired and the educators were still working in the classroom with children.
The licensee did not have documentation of consistently completing bi monthly staff observations as required.
In the Infant/Toddler 2 Classroom the emergency backpack was completely covering the emergency evaluation procedures and pictures. Nothing should be covering the procedures for emergency evacuations. -In the Infant/Toddler 1 classroom there was no evacuation pictures or procedures posted for emergency evacuations.
In the Infant/Toddler classroom the emergency backpack was left in the classroom while the group of children and educators were outside in the playground. The emergency backpack should be with the group of children at all times.

A review of staff records indicated the following: -The program did not have documentation of observing, recording bimonthly observations of the educators at the program. -One did not have documentation of registering annually with the Department of Early Education and Care.
A review of children's records indicated the following: -One did not have a current physical examination. Last physical examination expired on 1/5/24. -One did not have documented on an Individual Health Care Plan which educators were trained to administer the medication. -One did not have documented on their Individual Health Care Plan who trained the educators to administer the medication.
There were several areas of the wall in the Preschool Classroom that had paint chipping off the wall.
