The toddler group was outside in the playground. They had 7 children documented on the attendance as present. One child arrived at 8:45 a.m. and the educators did not document them present until 10:15 a.m. The program must maintain accurate attendance of the children at all times.
The gross motor room bathroom did not have diapering and toileting, and handwashing postings as required.

The gross motor room bathroom ceiling tiles, furniture, and equipment piled in there. -In the Pre-K bathroom there was a vacuum accessible to children. -In the Pre-K classroom the cabinet door under the sink in the classroom was not locked. There were bottles of soap and disinfectant accessible to children.

A review of attendance in the Pre-K Classroom indicated that there were 14 children documented present on the attendance. There were actually 15 children present. One child had arrived approximately 5 minutes prior and was not documented on the attendance. Programs must maintain accurate attendance.
The educators in the Pre-K classroom had a hot cup of coffee, a cold cup of coffee, and a soda on the table in the classroom accessible to children.

-A child receiving ABA services at the program did not have evidence of having a 3-month assessment completed. -A child receiving ABA services at the program did not have a current plan from the ABA program or the parent of the child. -The program did not have permission to allow ABA services to work with a child at the program. -The program did not have records of ABA visits to the program for the child as required.
A review of educator records indicated the following: -One educator did not have evidence of being teacher certified.
The program did not have evidence of a current Building Inspection from the local Building Department as required.
The gross motor classroom did not have posted evacuation procedures, emergency numbers, and allergy postings as required. -A child with medication for a known medical condition was documented to be in Two's 1 classroom. The child's meds and plans were not there. Educators stated the child had moved to the Preschool 1 classroom. Medication has to be readily available for the child in an emergency.
Two classrooms (Preschool 1 and Infant 1) did not have handwashing and toileting and diapering policies posted in the classroom and bathrooms. The educators posted the policies and procedures during the visit.

The following classrooms did not have complete evacuation procedures posted: Preschool 1, Gross Motor Room, Toddler 1, and Infant 1 classrooms. Written emergency/evacuation procedures must be posted at all exits in the program.

In the Toddler Two's 1 Classroom, the attendance had no written documentation of which children were present in the classroom. Programs must maintain accurate attendance at all times.
The gross motor room did not have emergency #'s posted and easily visible to all. Emergency postings must be in every room that children will be present in.
Emergency evacuation drills were not completed in the months of July and August 2025. Emergency evacuations must be completed and documented on a monthly basis.
A child had been transitioned to another classroom without taking their emergency medications with them. Medications have to be easily accessed for emergency purposes.

A review of medications and Individual Health Care Plans indicated the following: -One did not have a completed Individual Health Care Plan as required. -One had an IHCP that expired on 1/16/25. -One did not have a current medication consent form to administer medication. -One did not have documentation the possible side effects of the medication. -One did not have documentation of consequences of non-treatment. -One did not have documentation of who was trained to administer the medication. -One did not have documentation of who trained the educators to administer the medication.

The licensee did not have current building and fire inspections from the local building and fire departments.
The Two's 1 Classroom did not have a privacy protected list of children with allergies posted in the classroom.
The gym did not have posted the evacuation procedures in case of an emergency evacuation.
There was no secondary exit posted in the two's 1 Classroom as required.
