A review of five children's records indicated the following: -Five did not have a completed Developmental History form. -Three did not have a completed Emergency Authorization and Consent form signed, dated, and completed. -Five did not have evidence of a completed evaluation yearly. -Two did not have documentation of a completed physical examination yearly.
A review of staff records indicated the following: -Four did not have a completed and signed staff information form. -Three did not have documented two written references. -One did not have documentation of current First Aid Training. -Three did not have documentation of an interview. -Three did not have documentation of an application and or resume. -Three did not have evidence of completing a current physical examination. -Three did not have evidence of a current immunization from MMR. -Two did not have documentation of completing the program orientation checklist before working in a classroom with children.
In the infant/toddler classroom the play refrigerator and stove were not secured and easily able to tip over.
The non tar areas of the playground had large quantities of rocks that are accessible to infants and toddlers. Rocks are the size of quarters.
On 7/29/25 an unqualified educator was observed supervising three toddler/preschool children for approximately 1 hour. All educators assigned to supervise children must have the appropriate qualifications for the position they are working in.
A review of children's files indicated that none of the children had a current completed transportation plan as required.
The two cribs in the infant/toddler classroom did not have documentation of being an emergency evacuation crib.
This capacity increase visit inspection recorded no violations or advisories.
A review of emergency evacuations indicated that the licensee did not have documentation of completing an August emergency evacuation procedure.

A review of the children's records indicated the following: -Three did not have a completed Face Sheet. -Three did not have a completed Developmental History Form. -Three did not have a completed Authorization and Consent Form.
A review of all educator files indicated the following: -One did not complete the EEC Strong Start Training. -One did not have documentation of evidence that the educator has been immunized in accordance with the recommendations of the Department of Public Health. -One did not register annually with the Department of Early Education and Care in accordance with EEC policies and procedures.
The playground had some unknown vegetation growing in the playground and accessible to the children. The playground fence was not secured on the bottom. Children can lift it up and crawl under it. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.

The staff schedule posted was missing the following: CPR/FIrst Aid certification, Educator's full names and positions and certification.
The toys and books observed were appropriate for infant, toddler and preschool children, however, a school-age child was present during the visit and the program did not have enough materials to keep that child engaged in a developmentally appropriate activity.
The following was observed to be accessible to children: -A horizontal bookshelf sat on a white shelving unit and was not secure. -The bathroom contained low shelving with gloves, creams and diaper wipes. -Two telephone jacks were accessible and not covered. -In the office space, the white shelving units were not secure and tippable.

A review of first aid trainings indicated the following: -Three educators did not have current First Aid Training as required by EEC.
A review of the children's records indicated the following: -One did not have a parent signature or date on their developmental history form. -Two did not have current physical examinations from a certified physician. -One did not have documentation of an Individual Health Care Plan for a child with a (REDACTED) at the program. -One did not have a current medication consent form for the nebulizer medication.
The program did not have a current building inspection from the local building department.
A review of medication indicated the following: -One did not have current medication at the program for a diagnosed medical condition.
The program did not have posted in the children's bathroom a diapering and toileting plan to follow.
One medication has expired therefore it was not readily available.

A review of the evacuation drill log was reviewed and found: -A drill was missed in April 2023.
