A review of evacuation procedures indicated the following: -The Toddler/Preschool classroom did not have evacuation procedures post at each exit. -The Toddler 5 classroom did not have evacuation procedures posted at each exit.
Allergies, medical info, special diets, emergency medications; privacy protected
A review of Individual Health Care Plans indicated the following: -Four IHCP's were expired: 8/24, 9/24, 11/24, and 5/25. Children's IHCP's and medication consent forms must be current and accessible. -One did not have a current epi pen dispenser as required. Children's medication must be current, current medication consent form, current IHCP.
Indoor Space
A review of consent for medication forms indicated the following: -Four did not have current medication consent forms. They expired 5/25, 11/24, 9/24, and 8/24. All medications must be accompanied by written consent and healthcare practitioner authorization including proper administration of medication.
Children's Records
The bookshelf in the Preschool 1 classroom was not secured. The bookshelf is easily tippable.

A review of medications at the program indicated the following: -One epi pen had no name or prescription attached to it. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. -One (EXPIRED). -Three Individual Health Care Plans expired in August 2024.
In the Toddler II Classroom the rug is curling up and ripped. This is a hazard for the children.
Medication
The program did not have a current Building and Fire Inspection from the local town as required.
A review of medication at the center indicated the following: -One did not have a current not expired albuterol inhaler. -One had a medication consent form for Benadryl. There was no Benadryl at the program.
The first aid kit in the Toddler classroom did not have a CPR Mouthguard.

A review of three staff records indicated the following: -Two did not register annually with the Department of Early Education and Care. -Two did not have completed, dated and signed staff information forms.
The plug cover in the preschool classroom was broken and sharp on the top of it.
The preschool classroom did not have a second written evacuation plan and diagram posted.
The evacuation procedures were missing at the second egress in XXXX.
In XXXXXXXX, a child's diaper was changed and the educator did not wash the child's hands immediately. The educator handed the child to the second educator while she cleaned the changing table.

Individual Health Care Plan (IHCP) - R Apr 2025
The following was observed to be accessible to children: -Many windows in the classrooms have tape residue left over from taking hangings down. -Ceiling tiles were stained in TODDLER 1/TODDLER 4 (10 tiles), PRESCHOOL 2 (2 tiles), INFANT 1 (4 tiles), and TODDLER 2 (2 tiles). -In TODDLER 1, a package of wipes was in a drawer, paint is peeling in two spots in the middle of the blue wall, and a small section of the floor near the dish washer is missing and has cracked tiles. -In TODDLER 4, two bumps have formed under the tile in the center of the room and are a tripping hazard. -In PRESCHOOL 2, paint is peeling under the windows on the area close to the bathroom and the cabinet under the sink was unlocked (broken handle) and contained cleaning supplies and bleach. -In the BATHROOM BETWEEN PS and TODDLER 4, the drawers contained rolls of bags, loose garbage bags, bags of wipes, and bags of soap dispenser soap. -In INFANT 1, an outlet cover was missing from the outlet above the cribs and the cabinet below the sink was unlocked (sharp broken handle) and contained bags of wipes and plastic packaging for diapers (diaper count low). -The purple wall in the HALLWAY NEAR THE SIDE EXIT, has a section peeling off and the adhesive is accessible to children. -In PRESCHOOL 1, the paint is peeling on the yellow wall, the baseboard is peeling off where the pieces connect, the wall behind the trash bin was covered in drips and stains, two window blinds have broken slats, the Lego table was dusty, and the cabinet under the sink was unlocked and contains loose trash bags and soap dispenser bags. -In the PRESCHOOL 1 BATHROOM, loose trash bags and clear bags in a drawer were found. -In TODDLER 3, paint is chipping on the light blue wall and the changing table mat has a small rip in the center. -In PREKINDERGARTEN, the purple half wall has two sharp spots of plaster where screws have been removed, the floor in front of the refrigerator was dusty, the trash bin was not covered, and the Echo speaker cord was plugged in and the cord hung loosely (longer than 6 inches).
Staff Schedules were observed to be hanging on the walls on the outside of the classrooms, however, the schedule for XXXXXX was missing the second educator.

The following toxic substances were observed to be accessible to children: -In the multi-stall bathroom connected to Preschool 2, a drawer contained an aerosol air fresher. -In PREK, the lock on the cabinet below the sink was not secured and exposed multiple spray cleaners (secured during visit). -In the bathroom in PRESCHOOL 1, a drawer contained a bag of soap for the soap dispenser which states "Keep out of reach of children". -In the TODDLER 2 bathroom, the plastic drawers contained Desitin and Hello Bello diaper creams and a bag of soap for the soap dispenser, all of which state, "Keep out of reach of children".

The following was observed to be accessible to children: -In PRESCHOOL 2, there was a broken floor tile under the middle window, tape residue on the four low shelving units, and the toy car racetrack was visibly dusty. -In the multi-stall bathroom connected to Preschool 2, a drawer contained six rolls of baggies, a drawer contained loose plastic grocery bags, and another drawer contained loose garbage bags. The low cabinets on the left and right hand sides contained stains, crumbs, and dust. -In TODDLER 4 a loose pocket book was found in a wooden drawer, the light table was leaning against the wall under the cubbies and not secured and the half door leading into the Toddler 1 room had chipping paint along the top. -In TODDLER 1, a while bookshelf in the unlocked closet was not secured and considered tippable. -In PREK, the sleep mats were standing up not secured and considered tippable in the unlocked closet, the white board easel was closed and leaning against the wall not secured and considered tippable (fixed during the visit), a pair of adult scissors was in a drawer (moved during the visit), an opened box of gloves was on the counter next to the sink, the rubber bumper on the bottom of the purple wall is peeling off which is a tripping hazard and exposing glue on the wall, a hole is on the same wall, tape residue is on the round table in the play kitchen area, the play sink is dirty as evidenced by dust, the cord to the Boombox is hanging loosely from the outlet, and the bathroom contained a plunger and toilet brush. -In PRESCHOOL 1, the cabinets below the sink has lots of dust and crumbs, a staff pocket book was sitting on the sleep cots in the unlocked closet, there was a large spot of the windowsill where paint had peeling off, and the bathroom had a drawer containing a roll of baggies. -In INFANT 1, paint was chipping on the wall to the right of the dance bar, the contact paper to the right of the chipped paint was peeling off and sticky on the backside, the soft climbing blocks were frayed on all edges at the seams, -In TODDLER 3, the tracks for the sliding doors under the middle cabinet were dusty, tape residue was on the small square table, and the top of the Lego table was dusty. -In the atrium/common area a wooden slide greater than 18 inches in height and stacked climbing blocks also greater than 18 inches in height were being used by children without the required ground surface material around the use and fall zones. These items were removed from the space during the visit.
The following was observed and to be accessible to children in the outdoor space: -Large areas of chipping paint all along the building next to outdoor space, a basketball hoop not secured and tippable, and rust visible and chipping paint on the four white poles holding up the canopy.
Evacuation procedures were not posted at the following exits: PRESCHOOL classroom at exit infant room, PREK at both exits, PRESCHOOL at exit to PreK, INFANT 1, and TODDLER 3.
The Evacuation Log observed was missing the times of day the drills occurred.
A review of the medications was conducted and found one child's (REDACTED) has expired and another child's medication is not in the original container (box).

There was peeling paint throughout the program.
Several nap mats were torn on the seams. This left the soft padding inside exposed and unable to be cleaned.
Indoor Space
In one room, a hand soap dispenser had peeling tape. This left a sticky surface unable to be cleaned. One room had a broken cabinet door that was not securely attached.
