Children’s files were reviewed. It was discussed with the educator that three files did not indicate date of admission and age at time of admission. One file had a general permission that was not signed by the parent and the emergency card had incomplete sections. One file needs a current physical.

Children’s files were reviewed. Two files did not have current physicals, two files did not indicate lead screening, one file is due for the annual update, and discussed with educator having complete files for her own children as well due to subsidy.
Educator did not have proof of any attendance records. There was no daily attendance for today’s visit either. Educator indicated previous attendance records were ruined and could not be recovered. Reviewed importance of daily attendance records.
Exit from deck slider had icy floor.
The carbon monoxide in basement level did not go off when tested.
The swing set will need more wood chips to ensure there is the required 9 inches.
Review of children’s files was completed. Two files did not have general permission signed giving educator permission to take child off premises. The educator has to take children off premises due to yard not being approved.

Swing set observed in the backyard is not approved for use, it does not meet the requirements for appropriate ground surfacing and fall zones. Access to swing set must be prevented. At this time yard will not be approved for use, educator must have an alternative play space.
