A random review of children’s files was completed. There were three files that were missing physical description on first page of enrollment packet. Three files were missing lead screening. One file was missing date of admission and was on older version of enrollment packet.
There was a lamp on top of small table in dinning room that was easily moveable. There was access to beauty products in one of the bathroom drawers.
This monitoring visit inspection recorded no violations or advisories.
It was observed that outdoor climbing structure (slide measures above 18 inches) was repositioned and two of the sides did not meet the 6 foot distance fall zones.

Educator was observed changing a child’s diaper and not washing child’s hands afterwards.
It was observed that the educator washed hands in the kitchen sink after diapering. Handwashing after diapering must be done in a separate area from where food is prepared.

Review of educator’s attendance records, reflected some children not being signed out on 3/7/22, 3/18/22, and 3/28/22
A climbing structure was observed measuring above 18 inches, structure did not meet the required 6 feet fall zones all around as required.
Diapering mat was observed to be showing wear and tear. Mat needs to be replaced.

Review of files was completed. It was observed that some children had enrollment packets on old forms; educator needs to ensure all children have enrollment packets utilizing current EEC enrollment forms. One child did not have lead screening. Three children need an updated physical.
Emergency preparedness plan was not clear on how the educator will get children to designated place in case of an emergency evacuation.
Parent handbook was reviewed. Parent handbook does not reflect current capacity and does not have correct regional office information.
It was observed white fencing on left side was not sturdy and corner piece did not have the required 4 feet tall required for fencing. It was observed that the three steps leading to exit in TV room, although there is a gate at the top to prevent access, the opening of the side of the stairways poses a safety hazard. It was observed there was a a cage setup for the educator’s two dogs, and children were observed leaning over onto the cage, which was not sturdy and children could fall over the opening. The educator must install a proper barrier on the side of the stairs.
Educator was observed feeding infant. Educator was observed holding the bottle while the infant was on a bouncy chair. Educator must hold infant during feeding.
